JOB DESCRIPTION

Job Title: HCSCase Manager

Job Duties & Responsibilities

The Case Manager works in conjunction with the Service Coordinators from Local Authorities to ensure that quality services are provided to all individuals, ensuring that all of the individuals’ needs are being met in accordance with the individuals” and families/ LARs’ wishes and goals, and that all federal and state rules pertaining to the individual are followed as outlined in the Home and Community based standards.
Participates in Special Planning Teams (SPT) with the Local Authority and is responsible for generating required documentation regarding the Individual Plan of Care (IPC), Intellectual Disability/ Related Condition assessment (ID/RC), Implementation Plan (IP), etc.
Responsible for monitoring the progress or lack of progress on established goals and objectives included in the Implementation Plan.
Responsible for following any changes in the individuals’ lives and revising documentation in conjunction with Service Coordinator when necessary to revise current goals and outcomes based on the individuals’ needs and goals.
Meets established timelines for completion of required documentation as well as meeting timelines for sharing documentation with the Individual/LAR, Service Coordinators and other team members, ie, Behavior Analyst, Dietician.
Must have excellent time management skills
Must be capable of multi-tasking in a fast-paced environment
Effective written and oral communication skills

Qualifications

Valid Driver’s License

Knowledge of the HCS program Required

Minimum of 2 years working with Individuals with Developmental Disabilities

Any and all information discussed, obtained, written, reported, verbally communicated, or otherwise indicated regarding an individual’s HCS services, medical, financial, or other aspect of community living is protected information and must remain confidential. Refrain from discussing any aspect of an individual and/or his/her services outside of the company.

There is a ZERO TOLERANCE policy for failure to adhere to this standard.
