Job Overview

Join our compassionate healthcare team as a Hospice Case Manager, where you will play a vital role in providing holistic, patient-centered end-of-life care. In this dynamic position, you will coordinate and oversee comprehensive hospice services, ensuring comfort, dignity, and support for patients and their families. Your expertise in clinical case management, patient assessment, and interdisciplinary collaboration will help deliver exceptional quality care in a compassionate environment. This paid position offers an opportunity to make a meaningful difference while working within a dedicated healthcare community.

Responsibilities

Conduct thorough patient assessments to determine eligibility and develop individualized care plans aligned with hospice standards and patient wishes
Coordinate interdisciplinary team efforts, including physicians, nurses, social workers, chaplains, and therapists to deliver seamless hospice services
Manage clinical case documentation using Electronic Health Records (EHR) systems while ensuring compliance with HIPAA and health regulation policies
Provide ongoing patient care management by monitoring symptoms, vital signs, and medication adherence to optimize comfort and quality of life
Facilitate discharge planning and transition processes for patients moving between inpatient, outpatient, or home health settings
Collaborate with healthcare professionals across hospital medicine, post-acute care, emergency room, ICU, Geriatrics, Behavioral health, and other specialties to ensure comprehensive support
Educate patients and families about disease progression, symptom management, and available hospice resources to foster informed decision-making

Skills

Proven experience in hospice care or related fields such as critical care, ICU, trauma centers (Level I & II), or emergency medicine
Strong knowledge of medical terminology, physiology, and health regulation policies relevant to end-of-life care
Skilled in patient assessment, clinical case management, discharge planning, and utilization management within hospital or home health environments
Proficiency with Electronic Health Records (EHR) systems and medical records management while maintaining HIPAA compliance
Excellent client communication skills with the ability to collaborate effectively with healthcare professionals across multiple disciplines
Background in managing patients with complex needs including Geriatrics, Pediatrics (PICU experience), Infectious Disease Care or Behavioral Health considerations
Ability to navigate medical-surgical settings and acute care environments confidently while managing multiple priorities
