JOB SUMMARY

Under the supervision of a higher classified employee, conducts the intake process for the COC Coordinated Entry homelessness response system. Assists with shelter, prevention, and assessment referrals to other participating agencies. Assures eligibility compliance with all programs. Tracks and manages all files in the HMIS data system relating to homelessness.

EXAMPLES OF DUTIES

Essential Duties:

Under the supervision of a higher classified employee, conducts intake for the COC Coordinated Entry homeless response system, including answering calls to the HAWC phone line, assessing the urgency of the call, and making referrals to the appropriate agencies.

Assures program compliance with federal, state, and local regulations, as applicable. Enters data into database tracking systems and runs reports to monitor progress and for external funders.

Completes pre-assessment and intake for new clients. Prepares files and records. Meets with clients on-site when necessary and collects all required program documentation from client. Provides follow-up and assistance to new and existing clients.

Conducts case-level assessment of current client housing need to ensure compliance with Federal, State, and local policies, contract requirements, and fidelity with program goals and objectives.

Gather, organize, and analyze data for reporting and service improvement.

Develop and maintain relationships with partner agencies and attend community meetings, workshops, and seminars related to homelessness issues to remain current on community resources and needs.

Performs basic clerical tasks such as (but not limited to) answering the phone, data entry and retrieval, typing, copying, collating, and general filing.

Performs other related duties as assigned.

EMPLOYMENT QUALIFICATIONS

Knowledge, Skills, and Abilities:

Develop and maintain knowledge of the principles of the Housing First model.

Interpersonal and communication skills (written and oral) to interact effectively with individuals and agencies involved in related programs.

Ability to prepare clear, concise, and effective written materials.

Ability to collect, analyzing and evaluate varied information and data.

Ability to maintain accurate records and files and enter and retrieve information from a computer with sufficient speed and accuracy to perform the required work.

Ability to set priorities, using judgment to coordinate multiple projects and meet deadlines.

LICENSES/CERTIFICATIONS

Specified positions may require possession of a valid vehicle operator's license.

PHYSICAL DEMANDS

Duties require sufficient mobility to work in a normal office setting and use standard office equipment including a computer, vision to read printed materials and a computer monitor and hearing and speech sufficient to communicate in person or over the telephone.

EDUCATION

Associates Degree or equivalent in social work or closely related field.

EXPERIENCE

A minimum of two (2) years of experience working with low-income households on intake, counseling, or advocacy. Minimum of one (1) year of experience working in a call center.
