Who We Are
YSB's ultimate focus is to ensure all youth have access to safety, shelter and the opportunity to succeed. We do this through offering an array of services including a transitional living program for pregnant/parenting young women. We work to help youth repair foundations which have been jolted by abuse, neglect, homelessness and lack of resources, by guiding youth into safety, ensuring they have shelter, and teaching them how to overcome various obstacles. We know safety, shelter and providing success opportunities is key to establishing a bright future.
Youth Service Bureau delivers client-centered, trauma-responsive, and preventative services and support to youth experiencing safety and housing crises.
Summary
As the BTO (Beating the Odds) Case Manager, you will provide young people with intensive case management sessions using Trauma-Informed Care, Positive Youth Development, and Evidence-Based Practices. Your responsibilities will include helping youth develop self-sufficiency plans, coordinating and leading daily life skills groups, and offering resources to help youth achieve independence. You will provide aftercare services to youth who have completed the program. You will maintain a client-centered and solutions-based approach when working with clients. Additionally, you will plan and facilitate outings for residents.
Responsibilities
Client Engagement & Assessment
- Conduct thorough, trauma-informed intake and assessment of youth and young adults, to identify strengths, needs, and risk factors.
- Develop individualized service plans in partnership with clients that reflect goals in housing stability, education/employment, life skills, health needs, and social supports.
Case Management & Individualized Planning
- Create and manage service and case plans that define goals, interventions, timelines, and measurable outcomes.
- Coordinate access to services, including medical care, mental health and substance use services, education programs, and employment support.
- Provide ongoing case management through regular, strengths-based engagement, goal tracking, and adjustments to service plans, honoring youth voice, choice, and self-determination.
Connection to Services & Resources
- Link and refer youth to appropriate community resources, including education, vocational training, employment services, mental health care, physical health services, and benefits assistance to promote self-sufficiency.
- Facilitate youth access to supportive networks and community supports to foster positive permanent connections beyond the program.
Housing Stability & Independent Living Support
- Support youth in identifying and securing safe, stable housing options during and upon exit from the program, including connection to mainstream housing resources.
- Provide coaching on independent living skills — such as budgeting, money management, tenant responsibilities, and daily living competencies — to prepare participants for long-term stability.
- Support youth in transitioning from TLP to permanent or supportive living situations, including family reunification, independent living, and housing assistance programs.
Education & Economic Stability Support
- Assist youth with school enrollment, GED attainment, vocational training, employment readiness, job search support, and career planning.
- Monitor and document client participation and progress toward educational and employment goals.
Documentation & Compliance
- Maintain accurate, timely documentation in the HMIS system and other required reporting mechanisms to track progress toward performance standards (e.g., housing outcomes, employment, education, health).
- Compile and submit performance data to support compliance with funding requirements and grant reporting timelines.
Trauma-Informed & Culturally Competent Practice
- Deliver services using a trauma-informed, strengths-based approach that respects cultural, linguistic, and developmental diversity.
- Identify and respond to signs of trauma, human trafficking, exploitation, and other complex issues affecting clients and their children.
Advocacy & Client Support
- Advocate on behalf of clients with service providers, schools, landlords, employers, and social service agencies.
- Provide crisis intervention and support during emergencies, safety planning, and linkage to medical or legal services when necessary.
Collaboration & Community Engagement
- Build and maintain effective partnerships with community providers, health care agencies, housing authorities, educational institutions, and workforce programs that can strengthen service access for TLP clients.
- Participate in community meetings and interagency collaborations to enhance referral networks and program visibility.
- Plan and facilitate social activities, events, and outings for TLP clients.
Facilities & Environment of Care
- Conduct routine environment of care, unit, and safety checks across transitional housing settings to ensure spaces remain safe, sanitary, and program-compliant.
- Identify, document, and report facility concerns, submitting and monitoring maintenance requests and escalating urgent safety issues to ensure timely resolution.
- Support youth in maintaining safe and habitable living environments by reinforcing tenant responsibilities, cleanliness standards, and basic household safety practices.
Quality Assurance & Professional Development
- Maintain compliance with all grant conditions, agency policies, and FYSB guidelines.
- Attend ongoing training on best practices, trauma-informed care, human trafficking, and youth development to maintain competencies. This includes mandatory program and agency staff meetings.
- Develop individualized service plans in partnership with clients that reflect goals in housing stability, education/employment, life skills, health needs, and social supports.
Requirements
- Education: Bachelor’s degree preferred in psychology, Social Work,or a related field.
- Experience: 2 years of social work experience, preferred.
- Technology Skills: Basic computer skills, Office 365.
- Communication Skills: Must possess excellent written and verbal communication skills.
- Leadership/ Behaviors: Integrate trauma-informed care, positive youth development, and transition to independence skills into all client interactions while maintaining ethical boundaries and professionalism. Must be self-sufficient without requiring constant supervision

Other Important Information:
Reports To: TLP Supervisor
Core Hours: Sunday through Thursday, 12p-8p
Typical Work Week: Sunday-Thursday - in person
Travel: As needed for work-specific trainings and client needs.
