Full Job Description

MDS Remote

Job Summary:

Isle Health and Rehab is seeking a dedicated and skilled MDS Remote to join our team of healthcare professionals. As a vital member of our clinical team, you will play a critical role in ensuring the highest level of care and quality outcomes for our residents. We offer a collaborative and supportive work environment, with a focus on clinical excellence and genuine hospitality.

Responsibilities:

Conduct MDS assessments, RAI assessments, and Care Plans to ensure accurate and comprehensive documentation
Coordinate with interdisciplinary teams to develop and implement individualized care plans
Review and update resident records, ensuring compliance with regulatory requirements and company policies
Provide education and training to staff on MDS and RAI protocols
Participate in quarterly MDS QAPI meetings to review and improve MDS processes
Collaborate with other departments to ensure seamless communication and coordination
Perform other duties as assigned to ensure the delivery of high-quality resident care
