Position Summary

As a PRN Field Registered Nurse (RN), you will deliver high-quality, skilled nursing care directly in the patient’s home. You will play a vital role in promoting recovery, safety, and independence while ensuring accurate and timely documentation that supports excellent clinical outcomes.

Key Responsibilities

Provide skilled nursing care, including medication administration, wound care, IV therapy, and phlebotomy
Complete OASIS documentation in accordance with Medicare guidelines
Deliver holistic, patient-centered care following each individualized plan of care
Educate patients and families to support safe and effective self-care at home
Maintain availability for occasional emergency coverage of unexpected open visits
Maintain exceptional organization, communication, and clinical judgment

Qualifications

Location: Duval County and surrounding areas
Active Florida RN License – clear and in good standing
OASIS experience required
1+ year of Home Health nursing experience required
2 years of nursing experience preferred
Alzheimer’s & Dementia Training (or willingness to complete)
Ability to pass a Level 2 Background Screening *
Experience with Electronic Medical Records (Kinnser/WellSky preferred)
