Health Home Case Management Supervisor

Hybrid | New York City
$80,000–$85,000 annually, plus benefits

Lead a Team. Improve Care. Make a Measurable Impact.

ACCM is seeking an experienced and motivated Health Home Case Management Supervisor to lead our Care Management and Intake teams.

This role is ideal for a hands-on leader who enjoys developing staff, improving workflows, solving operational challenges, and helping members with complex health and social needs receive coordinated, high-quality support.

You will have the opportunity to influence how services are delivered, strengthen team performance, and contribute ideas that directly improve member outcomes and organizational growth.

What You’ll Do

Lead, coach, and support Care Managers and Intake Specialists.
Monitor team performance, caseloads, productivity, and service quality.
Oversee member assessments, enrollments, care plans, care coordination, and transitions of care.
Guide staff in resolving complex member situations and service barriers.
Strengthen member engagement and timely follow-up.
Ensure compliance with Health Home requirements, organizational policies, and applicable regulations.
Identify workflow gaps and implement practical process improvements.
Support quality-improvement initiatives and performance goals.
Collaborate with leadership, healthcare providers, health plans, and community partners.
Help build a positive, accountable, and solutions-oriented team culture.

What We’re Looking For

Bachelor’s degree in Social Work, Human Services, Public Health, Healthcare Administration, Psychology, or a related field.
At least two years of experience in Health Home, care management, case management, healthcare, or social services.
Prior supervisory, team-lead, or staff-development experience.
Strong communication, organization, judgment, and problem-solving skills.
Ability to manage competing priorities in a fast-paced environment.
Experience supporting staff with complex cases and performance expectations.
Working knowledge of care coordination, documentation, compliance, and member engagement.

Preferred Qualifications

New York State Health Home experience.
Experience using Foothold, Epic, or similar care-management systems.
Experience supervising Care Managers or Intake staff.
Bilingual Chinese language skills.
