Full Job Description

Position Summary and Job Duties

Primarily responsible for maintaining a welcoming, responsive access, linkage, and transition system for clients with complex needs. Oversees the effective implementation, operations, and outcomes of the access and transition Services. Areas of responsibility include overseeing supervisors who manage Crisis, Intake, and Outpatient functions. Works closely with other Program Managers in Human Services, and particularly within the Behavioral Health Division, to assure welcoming engagement to clients with complex needs. Responsible for quality assurance, personnel management within a trauma-informed framework, regulatory compliance, and collaboration with administrative services staff. Demonstrates leadership in collaboration with key community partners. Works as part of a team of Supervisors and Managers who share responsibility for the behavioral health continuum of care. Performs in a manner consistent with the HSD Leadership Charter, Professional Standards, and “Commitment to My Co-Workers.”

50% Oversees Program development and implementation to achieve identified outcomes

Assess program activity as it relates to achievement of key outcomes and apply quality assurance interventions to maximize effectiveness of program areas.
Implements and refines policies and procedures to assure compliance with relevant administrative codes and adherence to best practice standards.
Monitors program areas for adherence to established budget and make recommendations regarding budget adjustments.
Participates in strategic planning to improve the system of care for consumers and families in Rock County.
Provides leadership in the implementation of evidence based practices

20% Provides consultation and supervision to subordinate staff

Interviews, selects, trains, supervises, supports and evaluates assigned supervisory and direct care staff according to HSD and Rock County procedure.
As appropriate, reviews cases, applies clinical assessment skills, and makes determinations regarding appropriate course of action
Consults clinically on cases and provides feedback and direction to staff regarding assessment planning and interventions.
As appropriate, provides supervision to monitor staff performance and facilitate professional development
Promotes a client centered, recovery oriented philosophy by advocating for client rights and strength based approaches.

20% Collaborates with internal and external partners to assure coordinated care

Participates as a member of the Behavioral Health management team, working collaboratively with colleagues to assure a quality continuum of services for clients.
Develops relationships and contracts with internal and external partners to maximize access to services and community collaboration
Represents the HSD in the community as appropriate including education and community liaison duties with regard to behavioral health issues.
Communicates and collaborates with staff and supervisors across HSD divisions to assure coordinated efforts to serve clients.
Collaborates with HSD quality specialists to ensure that the program meets regulatory and best practice standards.

10% Other Duties as Assigned

Success Factors (KSA's)

Knowledge of clinical assessment, diagnosis, planning and intervention skills / standards for individuals with mental illness, AODA and co-occurring diagnoses, and specialized training and/or skill in the care of people with complex behavioral health treatment treatment needs.
Knowledge of clinical screening, assessment, level of care, and referral practices,
Knowledge of risk assessment and standards related to suicide, homicide and self injury for children, adolescents and adults.
Knowledge of best practice standards in the areas of behavioral health practice and service delivery
Knowledge of and support for principles of Trauma-Informed Care, Recovery-Oriented approaches, and Client-Centered care
Knowledge of applicable administrative codes and statutes including DHS 34, 35, 75, and Chapter 51among others
Ability to communicate effectively orally and in writing
Ability to establish and maintain effective working relationships with physicians, clients, administrative superiors, co-workers, the general public and community partners in a manner that supports collaboration.
Ability to collect, organize and utilize data for program improvement purposes
Skills in clinical supervision and supportive management of staff.
Excellent critical-thinking and problem-solving skills

Job Requirements, Education, Training & Experience

Masters degree, from accredited university, in psychology, social work, counseling or related field
Wisconsin LPC, LCSW, or LMFT license
Two or more years supervisory experience preferred
Five or more years experience working with individuals with significant behavioral health problems preferred

Essential Job Functions (physical elements, equipment and working conditions)

Physical Elements

Ability to stand, walk and sit.
Ability to exert up to 20 lbs of force occasionally or up to 10 lbs of force frequently

Equipment Use

Ability to use computer and computer keyboard.

Working Conditions

Ability to work non traditional hours and be on call after hours
Almost all indoor work but may be required to accompany staff to community and home based visits
A valid driver's license to get between various county buildings and attend meetings is required

Note: Potential exposure to physically or verbally abusive clients.
