Full Job Description

Run the program that keeps older New Yorkers at home

We are a licensed home care services agency (LHCSA) serving clients across New York City and Long Island, including a dedicated NYC Department for the Aging (DFTA) contract population. We are looking for an experienced home care leader to own that program end to end — the case portfolio, the coordinators who staff it, and the compliance record behind it.

This is a working management role. You will be in the exception queue and the case files as much as you are in leadership meetings, and you will report directly to our Director of Coordination.

What you will do

Own the program's performance — staffing continuity, EVV exception rate, and audit readiness across the full case portfolio.
Lead the coordinator team — hiring, onboarding, coaching, performance and coverage for a team of program coordinators.
Be the escalation point above individual coordinators for complex cases, program contacts, and cross-functional issues with Clinical, Compliance and Billing.
Own program compliance — documentation and reporting to DFTA / NYC Aging standards and contract requirements.
Hold the line on continuity — every authorized client staffed, every shift covered, no client left without care.

What we are looking for

3+ years of home care coordination or care management experience, including 1+ year supervising a team.
Working knowledge of HHAeXchange or a comparable scheduling / EVV platform.
Experience managing a team against service-continuity metrics.
Clear, calm communication with clients, families, aides and internal partners.
Comfort operating in a structured operating system (we run on EOS).

Nice to have

Direct experience with DFTA / NYC Aging programs.
Experience with NHTD, CDPAP, or managed long-term care.
Bilingual in any language spoken by our client population
