Home Health Physical Therapist
Signature Healthcare at Home, part of The Pennant Group, is seeking a full‑time Physical Therapist (PT) to join our Salem home‑health team. This role is ideal for clinicians who value autonomy, meaningful patient relationships, and the chance to deliver functional, high‑impact care directly in patients’ homes.

Key Responsibilities

PT Responsibilities

Conduct comprehensive physical therapy evaluations in patient homes.

Recommend and educate on durable medical equipment and home‑safety modifications.
Collaborate with interdisciplinary team members to ensure coordinated, patient‑centered care.
Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
Communicate effectively with physicians, caregivers, and internal team members.

Qualifications

PT Requirements

Current Oregon Physical Therapist license (or ability to obtain).
New graduates and experienced clinicians welcome.
Strong communication, assessment, and problem‑solving skills.
Ability to work independently in a field‑based environment.
Reliable transportation and valid driver’s license.
