Patient Care Liaison

Join Buckeye Commons Care & Rehabilitation's compassionate team as a Patient Care Liaison!

As a Patient Care Liaison at Buckeye Commons Care & Rehabilitation, you will play a vital role in ensuring the highest level of care and support for our patients and their families. You will serve as the primary point of contact, fostering open communication and collaboration among our care team members, patients, and patients' families. Your exceptional interpersonal skills, combined with your ability to navigate complex healthcare systems, will make a tangible difference in the lives of those we serve.

Responsibilities:

Collaborate with care team members to coordinate patient care, including scheduling appointments, tests, and procedures
Act as a liaison between patients, families, and healthcare providers, ensuring that concerns and questions are addressed in a timely and empathetic manner
Develop and maintain strong relationships with patients, families, and care team members to promote open communication and cooperation
Identify and resolve issues, escalating concerns to senior leadership as necessary
Maintain accurate and up-to-date records of patient interactions and care plans
Participate in quality improvement initiatives to enhance patient satisfaction and outcomes
