Full Job Description

Overview
We are seeking compassionate and dedicated Home Health Aides to provide exceptional in-home care to clients across various settings. This role is vital in supporting individuals with diverse needs, including seniors, those with disabilities, and patients requiring specialized medical attention. The ideal candidate will have a strong commitment to patient well-being, excellent communication skills, and experience working in home health or long-term care environments. Prior experience with assisted living, nursing homes, or hospice care is highly valued. This position offers an opportunity to make a meaningful difference in the lives of those you serve while working in a supportive and professional environment.

Duties

Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Monitor and observe patient conditions, reporting changes or concerns promptly
Administer medication and perform basic medical procedures such as catheter care and vital signs measurement
Support clients with memory care needs, including Alzheimer's and dementia care strategies
Document patient information accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems in compliance with HIPAA regulations
Assist with meal preparation and feeding, ensuring nutritional needs are met
Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment
Use assistive devices like Hoyer lifts safely to transfer patients when necessary
Provide emotional support and companionship to clients, fostering a caring environment
Support hospice patients with compassionate end-of-life care as needed

Qualifications

Prior experience in home health, assisted living, nursing homes, or long-term care preferred
Knowledge of medical terminology and documentation practices
Certification in CPR and first aid required; additional certifications such as hospice or dementia care are advantageous
Experience working with individuals with developmental disabilities or disabilities is a plus
Ability to work independently while following care plans and protocols
Strong communication skills for effective interaction with clients and healthcare teams
Ability to handle sensitive information in compliance with HIPAA standards
Physical ability to assist with mobility tasks and perform light housekeeping duties effectively
Valid driver’s license preferred for travel between client locations
