Nature of Work in this Position: The primary responsibility of the Health Management Coordinator is to assist our members in finding new ways to inspire healthier habits, serving to a great extent in assisting the Health Management Director meet the department mission. Will visit and work closely with: 1) member patients in the health fair and work site to promote wellness and help them realize their personal best, 2) employer groups to uphold health management program awareness and to assist them in the process of actively working towards better health within the membership by providing support, encouragement and education, and 3) community individuals/agencies to collaborate population health management initiatives and activities.
Illustrative Examples of Work:
- Responsible for contacting participants that are eligible for the program for enrollment and provides necessary health coaching to reduce or eliminate high-risk behaviors.
- Encourages adoption of habits that are conducive to a higher quality of life.
- Responsible for supporting operational aspect of the department to meet the organization’s customer requirements and satisfaction.
- Develop and conduct onsite health management program seminars and training courses for interested employees and family members.
- Develop and distribute health education materials to identified employees and family members.
- Develop customized care plan for program participants including goals and plan of action.
- Coordinate care with other health-related vendors for optimal patient care.
- Systematically identify opportunities impeding goal achievements, and performs or conducts interaction, interventions, activities to remove and/or reduce identified barriers;
- Participates in meetings, workshops and seminars for the purpose of conveying and/or gathering information required to perform functions and/or representing the company to external individuals and/or organizations;
- Prepares a variety of materials (e.g. reports, memos, letters, procedures, instructional and marketing pieces, assessments, etc.) for the purpose of documenting activities, providing written reference, and/or conveying information;
- Reports program-related assessment results for the purpose of providing the information needed to evaluate program performance and to create new and/or revised initiatives needed to meet the program’s objectives;
- Respond to inquiries from members, employer groups and external clients for the purpose of providing information and/or direction;
- Serve as a liaison with a variety of outside and community individuals/agencies for the purpose of conveying and/or receiving information and coordination program activities;
- Develop policies, procedures and programs related to Disease Management, Preventative Health, Health Education and Wellness in collaboration with members of the Medical Care Management team
- Supervise the planning, implementation and evaluation of the Company’s Disease Management, Preventative Health, Health Education and Wellness Programs
- Participate in quality improvement activities related to member outcomes and provider performance.

Minimum Experience and Training:
Any combination of education and experience providing the required skill and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to:
- A Bachelor’s Degree or higher in Health promotion, Health education, Nursing, Athletic training, Nutrition, or other health related field or Counseling or equivalent experience
- Minimum of one (1) year experience as a health coach preferred, or at least two (2) years’ progressive experience in an outpatient clinical setting or managed care environment
- Computer literate
- Proficient in the use of Microsoft Office (Excel, Word, PowerPoint, Outlook)
- Certification from an accredited professional health coach training program preferred.

Continuing Education and Training
Must be able to acquire a certification in field of expertise:
- Chronic Care Professional (CCP) Certification
- Certified Corporate Wellness Specialist
- HIPAA Certification
