Full Job Description

Job Title: Case Manager – Casa Paloma

Reports to: Program Manager

Status: Full-Time, Non-Exempt

Location: Midway City

SUMMARY

The Case Manager for Casa Paloma will work with clients in to provide quality service, case management, assist with attainment of housing, recovery support, and to ensure clients get the services they need. Primary responsibilities include outreach, engaging new clients, coordinating housing opportunities, supporting clients with health and recovery services, and facilitating community integration. This role requires strong leadership to ensure the provision of high-quality case management services and documentation to vulnerable individuals needing support to address barriers that may impact their health and attainment of stable housing. Case management services are expected to be provided at office, field and home settings on a regular basis, with documentation completed within twenty-four to forty-eight hours of delivery of service.

ESSENTIAL DUTIES & RESPONSIBILITIES

Client Care

1. This position requires on-site presence.

2. Meet with clients in their homes and/or community.

3. Design and implement daily activities that promote American Family Housing’s Program Model.

4. Provide individual support, coordination and linkages to community resources in the areas of job training, education, money management, income, benefits attainment, medical, legal, dental, substance use and mental health services.

5. Accompany clients to appointments when necessary.

6. Facilitate activities and groups that foster community and celebration, including therapeutic support groups and life skills groups.

7. Monitor monthly activity calendar.

8. Develop leadership skills in residents and encourage resident involvement in civic life through programs.

9. Assist clients in enrolling in mainstream benefits and obtain identification (i.e. California Driver’s License, Social Security Card).

10. Assist clients with improving home management skills, family coping, parenting skills, self-esteem, anger management, and stress management.

Documentation

1. Maintain proper records on case management and/or other activities as instructed utilizing the appropriate Electronic Health Record (EHR) System.

2. Enter case notes within 24 hours of services rendered.

3. Keep tenant files current through the computerized care management system.

Other

1. Advocate on behalf of the residents (public aid, social security, home services, etc.) and make referrals as necessary.

2. Work with a team to ensure client care standards are met; participate in wellness checks and unit inspection follow-up.

3. Work collaboratively with other staff to ensure appropriate levels of treatment/support.

4. Participate in conferences, workshops, special projects, staff meetings, and other duties as assigned.

5. Participate in organized strategies which enhance best practices and support enhanced quality of resident services.

6. Participate in weekly site team meetings as requested.

7. Attend all required training to ensure individual and professional growth.

8. Answer telephones and respond to email inquiries in the office.

9. Coordinate services with partner agencies as needed, and assist with the completion of forms including Disability and Medi-Cal eligibility, when applicable.

QUALIFICATIONS

1. Bachelor’s Degree in Social Sciences OR 2 years’ experience working with homeless population.

2. Current, valid California Driver’s License with an acceptable driving record and reliable vehicle.

3. Able to transport clients in your vehicle.

4. 1-2 years of experience working with homeless and / or low and mixed- income populations in addition to experience in child welfare, family work, and substance abuse.

5. Have an understanding of low-income and homeless individuals and their specific needs.

6. Knowledge of crisis prevention, intervention, goal setting, and resolution techniques; should be able to match sib techniques to particular circumstances and individuals.

7. Detail oriented with excellent time management, organizational, communication, interpersonal, and computer skills.

8. Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision.

9. Self-starter with the ability to stay ahead of the curve and thrive in fast-paced environment.

10. The ability to work well under deadlines and to multitask.

11. Proficient skills in Microsoft Office, including Word and Excel.

12. Excellent verbal and written communication skills, and critical thinking and problem-solving skills.

WORK ENVIRONMENT

2. Will necessitate working in busy and loud environments.

5. Regularly required to walk or drive to different local sites throughout the day.

6. Regularly required to sit, stand, bend, and occasionally lift or carry up to 35 pounds.

Pay: $23.08 - $24.69 per hour
