Location: Southern Minnesota Territory Coverage
Compensation: $95,000-$130,000 annually + Incentive Visit Pay
Schedule: Full-Time | Monday-Friday + Weekend and On-Call Rotation

You'll provide coverage throughout our Southern Minnesota territory, stepping in where your clinical expertise is needed while having the independence to manage your patients and your workday.

You're the RN Who Helps Keep the Entire Region Moving

A floating position isn't about randomly sending you wherever there's an opening.

It's about having an experienced clinical resource who can provide stability throughout a larger territory.

You may step in to cover an established caseload when another RN is away.

You may help a territory experiencing an increase in referrals.

You may provide additional support during a transition.

Or you may take ownership of admissions and visits where your experience can make the greatest immediate impact.

Your territory may change, but the expectation doesn't: provide exceptional care and leave things better than you found them.

What You'll Do

Provide skilled nursing visits throughout Southern Minnesota.
Perform comprehensive assessments and ongoing evaluations.
Assist with admissions, routine visits, and case management coverage.
Develop and update individualized plans of care.
Educate patients and caregivers regarding medications, disease management, safety, and care needs.
Identify changes in condition and coordinate appropriate interventions.
Support territories during staffing changes, PTO, leaves, and periods of increased census.
Collaborate with physicians, therapists, social workers, and other interdisciplinary professionals.
Complete accurate and timely documentation, including OASIS assessments.
Participate in rotating on-call responsibilities.

You're confident walking into unfamiliar situations.

You can quickly understand what's happening with a caseload and determine what needs your attention first.

Most importantly, you don't need constant direction to make good clinical decisions.

Home health experience and OASIS knowledge are highly valuable because you'll be expected to operate with a significant degree of independence in the field.

What We're Looking For

Active Minnesota Registered Nurse license required
Strong clinical assessment and critical-thinking skills
Ability to independently manage patients and changing priorities
Excellent case management and care coordination abilities
Strong patient and family education skills
Confidence communicating with physicians and interdisciplinary team members
Ability to adapt quickly to different territories and caseloads
Valid driver's license and reliable transportation
Home health experience strongly preferred
OASIS experience preferred
