Job Description

Permanent Supportive Housing- Case Manager

Job Purpose Through using Trauma-Informed Principles, Harm Reduction and a Housing First approach, PSH Case Managers deliver person-centered services designed holistically with the participant to address barriers, lif t strengths, and support health and housing. PSH Case Managers understand that being adaptive and flexible is critical to ensuring services improve participants' quality of life. Employing advocacy, they serve as a central point of contact in which care and services are coordinated. At every stage of the housing stabilization process, PSH Case Managers establish a positive rapport, beginning with outreach engagement to new referrals, home visits, and accompanying participants to appointments including but not limited to medical appointments. PSH Case Managers advocate for the needs and priorities of participants, ensuring their voices are heard, respected and lead how they choose to live. PSH Case Managers support a caseload of participants from diverse cultural and ethnic backgrounds with different levels of functioning and needs. The caseloads are reviewed and adjusted as needed to support fair and equitable work and alignment of available and changing resources.

Essential Duties & Responsibilities

Deliver holistic, participant-driven supportive services that sustain and retain housing, increase access to healthcare, including mental health and recovery services and address basic needs such as food, clothing and accessing mainstream resources.
Provide excellent trauma informed services for people who experience trauma, crisis, lack of personal support systems and may have a range of medical and behavioral health issues.
Demonstrate and cultivate positive ethics and boundaries that ensure respecting the rights, dignity and safety of residents while maintaining objectivity and professionalism.
Collaborate with property management staff, County and other partners to ensure successful housing stability for participants and provide advocacy and intervention to develop and implement strategies to mitigate concerns that jeopardize participant’s housing stability and prevent evictions.
Develop and implement individualized service plans in collaboration with participants based on comprehensive assessments that identify needs to develop goals, steps necessary to reach those goals, timeframes and monitoring for accomplishments and adjustments as needed.
Monitor the resident’s level of participation by providing wellness and safety checks as needed, such as when appointments are missed and frequency of engagement changes. Provide crisis intervention strategies when needed to promote the wellbeing of participants.
Tailor on-going contact and intensity of services delivered including the frequency of face-to-face home visits and accompanying participants at offsite locations when advocacy is needed.

Responsible for maintaining documentation of records that demonstrates the progress of services including eligibility and housing ready documents. Documentation can include program reports, internal and external database systems and quality of reporting must meet contractual requirements.

Provide and/or coordinate transportation and accompaniment to increase participant capacity for self sufficiency. Transportation may include travel from various locations within Los Angeles County.
Work collaboratively with Peer Specialist Team to support successful community events and activities, including identifying and outreaching to residents, assisting in set up, breakdown of event set-up and attending events to foster positive engagement.
Collaborate and participate within your Region with other community service providers, volunteers, and stakeholders. Participate in food distribution events to support residents.
Participate and contribute to agency meetings, supervision, training, and annual performance evaluations to be an active contributing member of the culture and growth of Housing Works.

Display an understanding of social forces such as racism, class, gender, poverty, and stigma of mental illness and be able to work with people from diverse backgrounds.
Passion for addressing the symptoms and root causes of systemic poverty.
Experience working in the homelessness services sector and/or with at-risk populations that include delivering services such as outreach and engagement services, intensive case management, crisis intervention, housing and landlord/tenant rights, eviction prevention.
Exercise good judgment in various situations and maintain a balance among multiple priorities. Effective communication, written and analytical skills including administrative, organizational abilities, effective listening, and interpersonal skills to successfully work both independently and in collaboration with a team.
Ability/willingness to give and receive constructive feedback to/from co-workers and program participants. 2 Relevant life experience that demonstrates achievement of lasting transformative change in one’s life where critical thinking, problem solving skills and collaboration can be qualified as transferable skills.
Experience in delivering services through the lens of trauma-informed principles, harm reduction and housing first modalities Strong computer literacy, proficient in Microsoft Office and ability to interface with internal and external database systems with attention to detail.
Employees working in Scattered Site properties must possess a reliable vehicle, insurance coverage, a valid California Driver’s License, and an acceptable driving record.
Employees working in Single Site Properties must have reliable transportation, a valid California Driver’s License, an acceptable driving record and be prepared to arrive to work on time, attend all necessary appointments and meetings. Possessing a reliable car with current insurance coverage is preferred for Single site.
Have or be willing to obtain CPR/First Aid Certification within 180 days of employment. Bilingual desired but not required.

Preferred: Baccalaureate degree in a related field with two years of experience working with at risk populations providing direct services. In lieu of a degree, relevant work experience demonstrating acceptable knowledge and transferable skills will be considered. Work Location This position provides in-person services at Permanent Supportive Housing Single Site buildings and/or.

Physical Requirements

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job: Knowledge and ability to drive a vehicle with a valid license. Walk and climb up stairs when necessary. 3 Carry, lift, and move up to 20 pounds. Able to reach with hands and arms. Handle, finger, grasp and feel office equipment, such as computers, printers, phones, postage machines, and fax machines. Sit, walk, and stand for periods of time. Speak, hear, and see throughout the day including have depth perception and color vision. .
