## About the Role

As a Service Coordinator, you are the central point of contact and the primary advocate for individuals with developmental disabilities. This is not a desk job; it's a dynamic and deeply rewarding role where you will build trusting relationships, empower individual choice, and make a tangible impact on people's lives.

You will guide individuals and their families through the New Mexico DD Waiver program, working collaboratively to develop, implement, and monitor comprehensive Individual Service Plans (ISPs) that reflect their unique goals and dreams. Your days will be spent facilitating team meetings, conducting home visits, and connecting individuals to a network of providers and community resources.

## Key Responsibilities

Facilitate person-centered planning meetings to develop and review Individualized Service Plans (ISPs).
Coordinate and monitor all waiver-funded services, ensuring the highest quality of care.
Advocate fiercely on behalf of individuals to protect their rights and ensure access to community resources.
Maintain accurate, timely, and compliant case files.
Serve as the primary communication link between the individual, their family, and all service providers.
Contribute to agency growth by participating in quality improvement initiatives and program development committees.

## Qualifications

Minimum Qualifications (Required):

High school diploma or GED required.
A minimum of two years of experience in case management or a related role within the I/DD field.
A strong desire to learn, develop professionally, and grow within a company. Proactively researches and identifies new or underutilized state and local resources, providers, and natural supports to build a robust and effective support team for each individual.
Excellent organizational, communication, and advocacy skills.
Must have a valid driver's license, reliable transportation, and the ability to pass all required background screenings.

Preferred Qualifications (Stand-Out Candidates):

Bachelor's degree in Social Work, Psychology, Human Services, or a related field
Bilingual (Spanish/English) preferred.
Experience using Therap, iTherapyDocs, or other EHR software common in the I/DD field.
Knowledge of Social Security, Medicaid, and other benefits available to individuals with disabilities.
