Full Job Description

PrimeCare is dedicated to providing comprehensive, patient-centered care for individuals of all ages—from children to senior adults. Our team focuses on improving health outcomes through chronic care management, preventive care, and personalized treatment plans.

Position Summary:
We are seeking a compassionate and skilled Family Nurse Practitioner to join our clinic team. The FNP will provide primary care services and chronic disease management for patients of all ages in a collaborative, clinic-based setting. This role emphasizes health promotion, disease prevention, and management of complex chronic conditions.

Key Responsibilities:

Provide comprehensive primary care and chronic care management to patients across the lifespan, including children, adults, and seniors.
Perform thorough assessments, including physical exams, patient histories, and diagnostic testing.
Develop, implement, and monitor individualized care plans for chronic conditions such as diabetes, hypertension, asthma, and cardiovascular disease.
Prescribe medications and therapies according to state scope of practice and clinic protocols.
Educate patients and families on disease management, preventive care, and lifestyle modifications.
Collaborate with physicians, nurses, and other healthcare team members to optimize patient outcomes.
Document patient care in the electronic medical record accurately and timely.
Participate in quality improvement initiatives and maintain compliance with regulatory requirements.

Qualifications:

Master’s or Doctorate degree in Nursing (MSN or DNP)
Current Family Nurse Practitioner (FNP) certification
Active [State] RN and APRN license
BLS/ACLS certification
Experience in primary care, pediatrics, geriatrics, or chronic care management preferred
Excellent communication, organizational, and clinical decision-making skills
