Description of role / Job Summary:
As part of the SLS (Supported Living Services) Management Team, the Case Manager (CM) / SLS Manager is responsible for overseeing the scheduling and support of Caregivers / Direct Service staff, supervising staff, leadership, staff development, training, performance improvement, implementation and management of key services in accordance with policies and procedures. Effectively interacts with the individuals, and their circle of support on the processes of assessment, planning, intervention, implementation and evaluation. Under the direction of the Program Director, the Case Manager (CM) / SLS Manager establishes and maintains good client relations and service by directing, coordinating and managing staff. Ensures coverage of shifts and serves as a backup for staff temporarily absent. It is expected that the Case Manager (CM) / SLS Manager will spend more than 50% of his or her time supervising and developing staff.
Essential Functions:
- Schedule staff, map out routes, and maintain a clean safe work environment
- Act as first responder in the field to emergencies and report special incidents according to applicable laws and regulations
- 24 hour on call response, as applicable
- Participate and provide training in the development of the program plan as well as client pre-annual assessment as needed
- Communicate on a regular basis with regional center, residential facilities, families and other services providers and agencies, as needed and or directed
- Responsible for the selection, training/coaching, reviews/evaluations, and supervision of assigned staff
- Plan, manage, coordinate, track, and evaluate assigned staff efforts to achieve the programs objectives and individuals
- Coordinate with the development and implementation of the individuals IPP objectives and or ISP objectives for which the program is responsible
- Maintain monthly calendar of activities for individuals/group, as pertain to assigned program
- Coordinates and shares the interdisciplinary treatment planning, individual assessment, intakes, meetings, and participates in client quarterly/semi-annual/annual meetings
- Performs other related duties and assignments as required
Tasks (partial list):
- Supervise, train, and provide guidance to the SLS Caregivers / Direct Services Staff in accordance with State, Federal, Regional Center and funding authority regulations, NorCal Individual Care, Inc personnel policies and the SLS Procedures & Operations Manual
- Participate as part of an interdisciplinary team in the development of each client’s Individual Support Plan (ISP); assess ongoing effectiveness of achieving objectives
- Supervise 6-8 clients/cases
- Assist with direct service for each client, when needed
- Serve as off-hours (weekends) contact on rotating basis with management team
- Actively help clients find permanent housing
- Help clients manage the clients’ IHSS workers
- Complete daily, weekly, monthly, and annual documentation accurately, and as required by NorCal Individual Care, Inc, State and Federal regulations; ensure timely submission
- Maintain client information, files & documents according to established standards and protocol at NorCal Individual Care, Inc
- Maintain requisite client documentation (i.e., calendars, ISPs, etc.) in clients’ homes, on company systems & company approved devices as appropriate
- Monitor condition of clients’ homes, and facilitate all repairs and required maintenance of a client’s home
- Responsible for the following of clients’ budgets and ensuring payment of bills
- Support maintenance of clients’ Medicaid eligibility by assisting staff with scheduling and attending all Medicaid appointments and providing proper documentation, when needed
- Maintain professional communication with families, Regional Center Service Coordinators, Case Managers, Behavioral Consultants, health care personnel, schools, landlords, employers, etc.
- Participate and/or facilitate in all meetings as needed
- Manage Clients schedules
- Manage Direct Services Staff Time-off & PTO
- Manage Direct Services Staff Time & Attendance (Time Card) Hours
- Assist the Program Director with Staffing & Scheduling
- Responsible for completing I-9 Forms for new hires
- Assist the Program Director in various projects
- Respond promptly and appropriately to on-call situations
Skills and requirements:
- Two years of experience in a human services delivery system, preferably in comparable programs serving developmentally disabled adults
- Completion of at least two years of post-high school education in a human services-related field or specialization
- Minimum of one year of experience supervising line staff
- Minimum of 6 months of professional experience in a SLS program preferred
- At least 18 months’ experience and/or training on working with clients with behavioral issues
- Proven leadership and team building skills
- Proficient in Computer applications
- Successful background check
- Valid CPR Certification
- Valid First Aid Certification
- Valid California Driver’s License
- Proof of Auto Insurance
- Reliable transportation
Education and Experience:
- High School diploma with two years experience in a human services delivery system, including at least 1 year in a comparable program or a Bachelor's degree in human services, or equivalent
- DSP 1 certification preferred
- CPR Certificate
- First Aid Certificate
- Two years minimum experience in a human services delivery system, preferably in comparable programs serving developmentally disabled adults
- Experience requirements can be waived if candidate possesses an Associate’s or Bachelor's degree in human services or equivalent
Work schedule:
As a Non-Exempt hourly position, the positions work schedule varies; can include weekend or overnight shifts when necessary. This position is not an 8:00 am to 5:00 pm position. Applicant must be willing to be available to their staff before and after regular business hours. The applicant must also be available to provide coverage when there is a staff shortage.
At the Program Directors discretion, this position is required to work at least 16 hours of direct services with clients per week.
