Full Job Description

Supportive Housing Case Manager

Open Door is seeking a Supportive Housing Case Manager to work alongside adults who have experienced chronic homelessness as they establish and maintain stable housing.

This is a relationship-centered position that combines case management, practical problem-solving, community-based work, resource navigation, and detailed documentation. Case Managers develop consistent relationships with participants and work alongside them to address barriers, connect with resources, pursue individual goals, and maintain stable housing.

What You’ll Do

Build consistent, supportive relationships with Supportive Housing participants through regular contact, home visits, and community-based engagement.
Provide ongoing case management and supportive services based on each participant’s needs, goals, and choices.
Assist with participant enrollment and housing-related needs.
Connect participants with benefits, medical and mental health care, substance use treatment and recovery resources, employment services, and other community supports.
Help participants identify and address challenges that could affect their housing stability.
Collaborate with landlords, healthcare and service providers, community partners, and other Open Door teammates.
Maintain accurate and timely participant files, case notes, service documentation, and HMIS data.
Complete documentation required by Open Door, HUD, and other funding and compliance requirements.
Develop working knowledge of permanent supportive housing requirements and Open Door’s Supportive Housing policies and procedures.
Participate in training and use approaches including trauma-informed care, harm reduction, motivational interviewing, de-escalation, client-centered care, and participant choice.
Assist with other housing-related responsibilities as needed.

Who We’re Looking For

We’re looking for someone who is compassionate, dependable, organized, comfortable working independently in the community, and able to build meaningful relationships with people who may be navigating significant challenges.

Relevant experience in case management, social work, homelessness services, behavioral health, housing, substance use services, or a related field is strongly valued. Relevant education, professional licensure, and experience will be considered when determining starting compensation.

LBSW or LMSW licensure is preferred but not required.

Position Requirements

Valid driver’s license and ability to maintain driving eligibility under Open Door policy.
Ability and willingness to conduct home visits, transport participants, and work throughout the community.
Ability to communicate effectively in person, by phone, email, and electronic systems.
Ability to learn and consistently use electronic documentation and data systems, including HMIS.
Ability to occasionally lift or carry up to 50 pounds and assist with practical housing-related tasks.
Ability to complete required training related to supportive housing, participant services, safety, documentation, and compliance.
No professional license is required for this position.

Compensation & Employment

Full-time, exempt position
$48,000–$54,000 annually
Starting compensation within the range is based on relevant experience, education, licensure, and other job-related qualifications.
In-person position with regular work in participant homes and other community settings.
Reports to the Supportive Housing Director.
