Full Job Description

Community First Health Centers is looking for a Behavioral Health Clinician - Adolescent in the Algonac area. This position works directly with adolescents and their families in a school based setting.

POSITION SUMMARY:

Provide mental health and social work services assisting children, young adults, their families and the school community with emotional, social, financial and environmental issues that create barriers for healthy functioning. The Mental Health Therapist will work collaboratively with all members of the health team developing comprehensive treatment plans to meet identified needs within a healthcare or school-based setting. Mental Health Therapist professionals are expected to provide the highest quality of services in accordance with agency policy and professional standards and expected to work independently with minimal supervision.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Administer screening tools to determine need areas and diagnostic implications
Complete diagnostic assessment and intake screening to determine eligibility and treatment intervals
Develop individualized and comprehensive treatment plans for established clients
Be responsible for parent contact when necessary and appropriate; protecting client confidentiality while developing positive communications and relationships
Provide group therapy to identified clients, establishing number of structured sessions, documenting progress/regression, identifying and defining treatment goals/outcomes
Complete documentation as required noting progression/regression in EMR
Administer suicide assessments, create safety plans, and facilitate psychiatric hospitalizations
Provide clinical consultation, crisis intervention, and support to school staff.
Provide crisis intervention as indicated with established clients facilitating crisis supports after hours
Independently manage complex and difficult cases, utilizing strong assessment and diagnostic skills
Maintain client confidentiality per Mental Health Code Act of 258 of 1974, HIPPA and FERPA policy and procedures
Educate clients, staff, community and family members regarding symptoms of mental health concerns increasing awareness and decreasing stigma of mental health issues
Complete ESB following each billing contact documenting for insurance compliance
Report quarterly statistics of unduplicated clients, positive depression screens, diagnostic implications, including treatment plans, quarterly reviews and rescreens to Michigan Department of Health and Human Services promoting CQI compliance
Follow the rules and regulations for the social work profession as set forth by the State of MI, Department of Licensing and Regulation, and NASW Code of Ethics.

QUALIFICATIONS:

Requires Master’s Degree in Social Work, Master’s level (fully licensed-LMSW) or , Master’s Level professional counselor from an accredited University
Must be fully licensed LMSW or LPC in the State of Michigan
3 to 5 years of extensive social work experiences demonstrating considerable knowledge of target population preferred.
Qualified to receive third party insurance payments for mental health services

ESSENTIAL ABILITIES FOR POSITION:

Ability to read, write & communicate in English; follow written and verbal instruction and understand basic mathematics and figure checking
Ability to interpret and apply procedures, rules, technical information, instructions, manuals to complete tasks accurately and on schedule
Ability to learn from directions, observations, and mistakes; and apply procedures using good judgment
Ability to use discretion and maintain sensitive and confidential information
Ability to work independently or part of a team; ability to interact appropriately with others such as School officials, employees, vendors and the general public
Ability to work under supervision, receiving instructions/feedback, coaching/counseling and/or action/discipline
Adaptability to change in the work environment, managing competing demands and ability to deal with frequent changes, delays or unexpected events
Knowledge of principles and processes for providing customer and personal services including customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction
Proficient skills using Microsoft Office programs including, but not limited to: Word, Excel, Access, and Outlook required. Proficiency may be tested

CONDITIONS OF EMPLOYMENT:

Prior to starting employment, the candidate must complete a satisfactory background check. The background check is understood to encompass all forms of pre-employment screening, including but not limited to: drug screen, physical, criminal background, Central Registry clearance, driving record, sex offender registry, education, training and licensing verification, school and employment records, personal traits, integrity and professional and personal references. Candidate must successfully complete an annual background check throughout employment in the position in order maintain satisfactory qualifications.
Possess a valid State of Michigan Operator's License, an operable insured automobile for authorized travel, and maintain eligibility to drive as per the County's vehicle policy throughout employment in this position
Successfully complete all required training as mandated by the agency, department and state regulations
