Full Job Description

TITLE: Service Coordinator

REPORTS TO: Administrator

Non-Exempt

PURPOSE OF POSITION:

The Service Coordinator is responsible for ensuring that elderly residents, particularly those who are frail, disabled, or otherwise at risk, are connected with the supportive services and resources necessary to maintain their independence and remain safely in their apartments. The Service Coordinator provides individualized case management and resident support, assessing needs, coordinating appropriate services, facilitating referrals, and advocating for residents as needed.

The Service Coordinator is entrusted with the administrative authority, responsibility, and accountability necessary to effectively carry out the duties and responsibilities of the position in accordance with organizational policies, procedures, and applicable regulations.

ROLE and RESPONSIBILITIES:

1. Assists and advises residents and families of the services, which may be necessary to maintain an independent lifestyle.

2. Promotes wellness activities for all residents.

3. Educates residents, families, and staff on available community resources and help them obtain benefits they are eligible for.

4. Educates the residents on service availability, application procedures, residents’ rights, etc. both individually and as a group.

5. Advocates and may negotiate on behalf of residents for adequate, timely and cost effective provision of services.

6. Assists residents with navigating administrative and personal matters, including interpreting correspondence and completing forms when they are unable to do so independently.

7. Coordinates utility, telephone, medical, and other service arrangements, including establishing and managing payment schedules.

8. Assists residents in resolving errors, discrepancies, or misunderstandings related to Social Security benefits and earnings, insurance billing, and survivor or death benefits.

9. Provides support with end-of-life and bereavement-related matters, including coordinating funeral arrangements and connecting residents and their families with hospice care, bereavement counseling, and other supportive services.

10. Helps residents navigate complex administrative processes and resolve other bureaucratic or service-related issues, serving as an advocate and resource to ensure appropriate assistance and access to available services.

KEY CHARACTERISTICS:

Highly focused and strong attention to detail

Ability to work with frequent interruptions

Excellent communication skills written in oral

EDUCATION/EXPERIENCE/QUALIFICATIONS:

1. High school diploma or equivalent required; experience in social work, human services, gerontology, healthcare administration, or a related field is preferred.

2. Previous experience working with older adults, preferably in assisted living, independent living, senior housing, healthcare, social services, or a related setting. Is preferred.

3. Knowledge of local, state, and federal resources available to older adults, including Social Security, Medicare/Medicaid, insurance programs, transportation services, healthcare resources, and community-based support services is preferred.

4. Proficiency in Microsoft Word and Excel is required.

Must be able to push, pull, move, and/or lift a minimum of 50 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 10 feet.
