Job Title: Physical Therapist (PT / DPT) | Southern Minnesota | $15,000 Full-Time Sign-On Bonus

Company: Legacy Home Care / Home Health Care, Inc.

Location: Southern & Southwestern Minnesota | Regional Travel Required

Compensation: $110,000-$210,000 Annually + $15,000 Sign-On Bonus for Full-Time PTs/DPTs

Schedule: Full-Time or Part-Time | Monday-Friday Only | No Weekends | No Holidays | Minimum 2 Days / 12 Visits Per Week

Legacy Home Care and Home Health Care, Inc. are expanding throughout Southern and Southwestern Minnesota, and we're looking for Physical Therapists who want to build something substantial closer to home.

This isn't a position where you'll sit inside the same clinic waiting for the next patient to arrive.

You'll become the PT your territory depends on.

Build your caseload. Serve communities that need greater access to home-based therapy. Work independently. And have the opportunity to earn $110,000-$210,000 annually based on the career you want to build.

For Full-Time PTs/DPTs, we're also offering a $15,000 Sign-On Bonus.

If you enjoy variety, independence, and being out in the community, regional home health can offer a completely different kind of PT career.

You'll travel throughout an assigned portion of Southern or Southwestern Minnesota, bringing Physical Therapy directly to patients in their homes.

Some driving comes with the territory.

But so does something else:

Freedom from spending every day inside the same four walls.

You'll organize your caseload, travel between patients, make clinical decisions independently, and have an experienced interdisciplinary team behind you when you need them.

For the right therapist, that's not just a different job.

It's a better way to work.

Your Expertise Can Matter Even More Outside the Metro

Patients throughout Southern Minnesota don't always have the same access to healthcare resources available in larger metropolitan areas.

For someone recovering from surgery, hospitalization, illness, or injury, traveling to outpatient therapy multiple times each week may not even be realistic.

So you bring the therapy to them.

You'll help patients regain strength, improve balance, safely navigate their homes, reduce fall risk, and rebuild the functional abilities they need to remain independent.

Your clinical expertise has an immediate, practical impact.

What You'll Do

Provide in-home Physical Therapy evaluations and treatment visits.
Complete Start of Care assessments and admissions.
Develop individualized plans of care.
Address strength, balance, gait, transfers, mobility, endurance, and functional independence.
Identify fall risks and barriers to safely remaining at home.
Educate patients, families, and caregivers.
Supervise and collaborate with Physical Therapist Assistants.
Coordinate care with nurses, physicians, therapists, and other interdisciplinary professionals.
Monitor patient progress and modify plans of care as appropriate.
Complete accurate and timely documentation and OASIS assessments.

You Don't Have to Come From Home Health

Home health and OASIS experience are valuable, but we're also interested in strong Physical Therapists coming from:

Acute Care
Inpatient Rehabilitation
Outpatient Orthopedics
Skilled Nursing
Transitional Care
Senior Living
Hospital-Based Rehabilitation
Geriatric Therapy

What matters is whether you're clinically confident, independent, organized, and comfortable taking ownership of your territory and patients.

What We're Looking For

Current Minnesota Physical Therapist license.
PT or DPT.
Strong clinical assessment and decision-making abilities.
Ability to independently manage a regional caseload.
Comfort traveling throughout an assigned Southern or Southwestern Minnesota territory.
Valid driver's license and reliable transportation.
Excellent patient and caregiver communication.
Home health experience preferred.
OASIS experience preferred.
