Job Title: Licensed Practical Nurse (LPN) Visit Nurse | Home Health Freedom & Flexibility
Company: Home Health Care, Inc.
Location: Southwest Minnesota + 35 mile radius from home.
Schedule: Full-Time | Monday-Friday + On-Call and Weekend Rotation
Compensation: $30-$38 per visit + Overtime Opportunities

What You'll Be Doing

Complete skilled nursing visits and ongoing assessments
Administer medications and treatments according to physician orders
Monitor changes in condition and communicate updates to the care team
Educate patients and families on disease management and safety
Promote comfort, healing, and independence

Be A Trusted Partner

Collaborate closely with RN Case Managers and interdisciplinary teams
Support individualized care plans
Maintain accurate documentation
Help prevent unnecessary hospitalizations and improve outcomes

What You Bring

Required

Active Minnesota LPN license
Minimum one year of clinical experience
Strong assessment and communication skills
Ability to work independently with confidence
Reliable transportation and willingness to travel

Preferred

Home health, hospice, TCU, or long-term care experience
Passion for relationship-based nursing
