Job Title: Client Care Coordinator
Job Type: Direct Hire
Status: Full-Time
Work Arrangement: Primarily In-Person
Schedule: Monday–Friday | 8:00 AM–4:30 PM
Location: White Bear Lake, MN
Compensation: $65,000–$80,000 annually
Reports To: Leadership/Operations
Travel Requirements: Travel throughout Anoka, White Bear Lake & Chisago Lakes areas

Position Overview:
The Client Care Coordinator is a relationship-driven professional responsible for helping seniors and other adults remain safe, comfortable, and independent at home. This position serves as a primary point of contact for prospective and current clients, their families, and community referral partners.

This role conducts client assessments, develops personalized Plans of Care, coordinates services with the care team, monitors client satisfaction, and recommends changes as clients' needs evolve. The Client Care Coordinator also supports business growth by developing trusted relationships with hospitals, transitional care facilities, senior communities, healthcare professionals, and other referral partners.

The ideal candidate combines compassion with sound professional judgment. They listen carefully, communicate clearly, take ownership of their responsibilities, and help families make informed care decisions, often during challenging or emotional circumstances.

Key Responsibilities:
Client & Family Relationships

Meet with prospective clients and their families in their homes, hospitals, transitional care facilities, senior communities, the office, or virtually.
Listen carefully to each client's concerns, priorities, family dynamics, and care goals.
Explain available home care services clearly and recommend appropriate solutions using a consultative, low-pressure approach.
Help clients and families feel heard, supported, and confident in their care decisions.
Maintain regular communication with clients and authorized family members throughout the care relationship.
Respond promptly and professionally to questions, concerns, and changes in circumstances.
Advocate for each client's safety, dignity, independence, and overall well-being.

Care Assessment & Coordination

Conduct client assessments and develop individualized Plans of Care.
Clearly communicate client needs and service expectations to caregivers, nurses, operations staff, and other members of the care team.
Monitor client satisfaction, service quality, safety concerns, and changes in care needs.
Conduct follow-up visits and recommend service adjustments when additional or different support would improve client outcomes.
Visit clients during hospital or transitional care stays, when appropriate, to evaluate changing needs and support a safe return home.
Collaborate with clients, families, and internal team members to resolve concerns and develop practical solutions.
Maintain complete, accurate, and timely records in client-management, care-management, and CRM systems.
Protect client confidentiality and follow all applicable company policies and professional standards.

Community & Referral Development

Develop and maintain productive relationships with social workers, discharge planners, rehabilitation teams, senior communities, physicians' offices, and other professional referral sources.
Represent the organization at networking meetings, health fairs, community events, educational programs, and professional gatherings.
Follow up consistently with prospective clients, families, and referral partners.
Educate community members and professional partners about home care services and available support.
Identify opportunities to connect families with appropriate services earlier in the care process.
Document referral activity, prospective-client communication, and relationship-development efforts in the CRM.
Work collaboratively with leadership to support client-service and community-development goals.

Qualifications:
Required

Associate degree or an equivalent combination of education and relevant professional experience.
At least three years of experience in senior care, home care, healthcare, care coordination, client services, community relations, consultative sales, or another relationship-centered role.
Working knowledge of aging, healthcare, or common senior-care needs.
Demonstrated ability to build trust with clients, families, colleagues, and professional partners.
Strong verbal, written, interpersonal, and presentation skills.
Ability to discuss sensitive health, safety, family, and financial concerns with confidence and discretion.
Strong listening skills and a genuine curiosity about people and their needs.
Sound judgment and respect for client privacy, professional boundaries, and confidentiality.
Strong organizational, documentation, time-management, and follow-through skills.
Ability to take initiative, manage competing priorities, and work independently while contributing to a team.
Proficiency with Microsoft Office and the ability to learn CRM and care-management platforms.
Valid driver's license, reliable transportation, and willingness to travel throughout the assigned service area.

Preferred

Bachelor's degree in healthcare administration, social work, human services, business, marketing, or a related field.
Experience in home care, senior living, healthcare, social services, or care management.
Experience conducting client assessments or developing Plans of Care.
Experience working with hospitals, rehabilitation centers, social workers, discharge planners, or senior communities.
Experience delivering presentations, educational programs, or community events.
Experience developing professional referral relationships.

Working Conditions:

Primarily in-person position involving office work and travel to client homes, hospitals, transitional care facilities, senior communities, and community events.
Regular travel throughout the Anoka, White Bear Lake, and Chisago Lakes service areas.
Typical working hours are Monday through Friday, 8:00 AM–4:30 PM.
Occasional care conferences, professional meetings, trade shows, community events, client intakes, or other responsibilities may occur outside regular business hours or on weekends.
Regular interaction with clients, families, healthcare professionals, referral partners, caregivers, and internal team members.
