Full Job Description

Primary Care Physician (MD/DO)

Reports To: Chief Medical Officer or Medical Director

Position Summary

The Primary Care Physician provides comprehensive, high-quality primary medical care to patients of all ages within a Federally Qualified Health Center (FQHC). The physician delivers patient-centered, evidence-based care emphasizing prevention, health promotion, chronic disease management, and coordination of care. Services are provided regardless of a patient’s ability to pay and are delivered in accordance with FQHC regulations, HRSA requirements, and organizational policies.

The physician works collaboratively with advanced practice providers, nurses, behavioral health clinicians, pharmacists, social service staff, and other members of the care team to improve the health outcomes of underserved and medically vulnerable populations.

Essential Duties and Responsibilities

Clinical Responsibilities

Provide comprehensive primary care services including preventive, acute, and chronic disease management.
Diagnose and treat illnesses, injuries, and chronic medical conditions.
Perform appropriate physical examinations and medical assessments.
Order, interpret, and review laboratory tests, diagnostic studies, and imaging.
Develop and implement treatment plans consistent with current evidence-based guidelines.
Prescribe medications and monitor treatment effectiveness.
Perform office-based procedures within the physician’s scope of practice and privileges.
Provide health maintenance services including immunizations, screenings, and preventive care.
Coordinate referrals to specialty care, hospitals, and community resources.
Participate in care transitions and follow-up after emergency department visits and hospitalizations.
Maintain appropriate medical records in the electronic health record system.

Population Health and Quality Improvement

Participate in quality improvement activities and clinical performance initiatives.
Assist the organization in achieving quality benchmarks, UDS measures, value-based care initiatives, and patient outcome goals.
Promote preventive care and chronic disease management initiatives.
Support patient-centered medical home principles and team-based care.
Utilize population health tools to identify and manage care gaps.

Collaboration and Team-Based Care

Collaborate with nurse practitioners, physician assistants, nurses, behavioral health providers, pharmacists, care coordinators, and social service staff.
Participate in case conferences and interdisciplinary care meetings.
Support integrated behavioral health and substance use disorder treatment services.
Serve as a collaborating or supervising physician as required by state law and organizational policy.

Administrative Responsibilities

Complete medical documentation in a timely manner.
Review and respond to laboratory results, diagnostic reports, refill requests, and patient communications.
Participate in provider meetings, staff meetings, and committee assignments as requested.
Maintain compliance with organizational policies, accreditation standards, and regulatory requirements.
Participate in peer review, quality assurance, and risk management activities.

Community and Mission Responsibilities

Support the mission, vision, and values of the health center.
Provide compassionate and culturally competent care to underserved populations.
Promote access to care regardless of a patient’s financial status, insurance coverage, or social circumstances.
Participate in community outreach and health promotion activities as appropriate.

QualificationsEducation

Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.

Licensure and Certification

Current unrestricted license to practice medicine in the State of Illinois.
Board certified or board eligible in Family Medicine or Internal Medicine.
Current DEA registration.
Current Basic Life Support (BLS) certification; Advanced Cardiac Life Support (ACLS) preferred.

Experience

Previous experience in primary care preferred.
Experience working in a community health center, rural health clinic, or underserved setting preferred.
Experience with electronic health records preferred.

Knowledge, Skills, and Abilities

Strong clinical diagnostic and treatment skills.
Knowledge of evidence-based clinical guidelines.
Commitment to patient-centered and team-based care.
Excellent communication and interpersonal skills.
Ability to work effectively with diverse patient populations.
Knowledge of FQHC regulations, quality measures, and value-based care initiatives preferred.
Ability to maintain confidentiality and comply with HIPAA requirements.

Physical Requirements

Ability to sit, stand, walk, bend, and perform routine clinical activities.
Ability to use computers and electronic medical records systems.
Ability to occasionally lift or move up to 25 pounds.

Work Environment

The work environment is primarily a clinical outpatient setting. The physician may occasionally travel between clinic locations, hospitals, or community sites as needed.

Pay: $200,000.00 - $255,000.00 per year
