Position Summary:
We are seeking a compassionate and dedicated Home Health Registered Nurse to join our team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will work closely with patients and their families to develop personalized care plans, monitor health conditions, and provide education on managing their health. This position requires a strong understanding of critical care, long-term care, and various medical practices.

Key Responsibilities:

Conduct initial patient assessments and Start of Care (SOC) visits.
Develop and implement individualized patient care plans based on nursing assessments and medical orders.
Perform follow-up visits, evaluations, recertifications, re-assessments, and discharges as needed.
Administer medications, treatments, and injections in accordance with prescribed orders and agency protocols.
Educate patients and caregivers on disease processes, medications, and care procedures to promote patient health and self-management.
Collaborate and communicate effectively with physicians, therapists, case managers, and other members of the healthcare team.
Document all patient care activities accurately, completely, and timely in accordance with agency policies and regulatory standards.
Monitor patient progress and make necessary adjustments to the plan of care in coordination with the physician.
Maintain compliance with state, federal, and agency regulations and policies.
Travel to patients’ homes within assigned service areas.

Qualifications:

Valid and active Registered Nurse (RN) license in the state of California.
Minimum of one (1) year of clinical nursing experience; home health experience preferred.
Strong clinical skills and critical thinking ability.
Excellent organizational, communication, and interpersonal skills.
Ability to work independently with minimal supervision.
Valid driver’s license, reliable transportation, and proof of auto insurance.
Proficient in the use of electronic medical records (EMR) systems.

Job Types:

Competitive per-visit rates commensurate with experience and visit type.
