Overview

Join our compassionate team as a Home Caregiver and make a meaningful difference in the lives of individuals requiring personalized care and support. In this vital role, you will provide attentive, respectful, and professional assistance to clients in their homes, promoting independence, safety, and well-being. Whether supporting seniors, individuals with disabilities, or those recovering from illness, your dedication will help enhance their quality of life through attentive caregiving and compassionate service.

Duties

Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
Monitor patient health by observing vital signs and recognizing changes in condition; document findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
Assist with medications safely following prescribed schedules and medical terminology guidelines; assist with medication management
Support clients with specialized needs such as Alzheimer's care, dementia care, memory care, hospice care, or end-of-life assistance
Perform patient transfers using proper methods or patient transfer techniques to ensure safety for both client and caregiver
Maintain patient confidentiality in compliance with HIPAA regulations; document medical records and care activities thoroughly and accurately
Provide light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment

Skills

Strong patient observation skills to detect changes in health status promptly
Experience working with individuals with developmental disabilities or geriatric patients in assisted living or nursing home settings
Knowledge of home healthcare regulations and medical documentation standards
Proficiency in using EMR/EHR systems for accurate record keeping and communication
Ability to perform CPR and First Aid confidently; trained in basic life support techniques
Familiarity with patient transfer methods, including the use of Hoyer lifts and other mobility aids
Excellent client communication skills to build trust and ensure personalized care plans are followed effectively
