Full Job Description

Fatherhood Program Case Manager

Full-Time | Lake County (some time spend at Cleveland office)

About the Role:

Passages is seeking a dedicated STEPS Program Case Manager to support participants working toward stronger family functioning, employment, and long-term stability. This role includes one-on-one coaching, goal setting, case management, and facilitating fatherhood or life-skills programming. Evening availability is required to meet participant needs and assist with evening workshops. (1-2 days per week at Cleveland office)

(Supervision may be available for social work students earning bachelor's or master's degree)

Key Responsibilities:

Develop and update Individual Development Plans (IDPs) with participants.
Meet with participants weekly (in-person or virtual) for coaching and progress reviews.
Complete intakes, assessments, and collect required documentation.
Provide employment support including resumes, interview prep, and job search guidance.
Facilitate or support fatherhood and life-skills workshops.
Connect participants with community resources such as housing, transportation, training, or behavioral health services.
Use a strengths-based, trauma-informed approach to support healthy family functioning.
Support participant recruitment, retention, and engagement.
Collaborate with program mentors and staff to coordinate services.
Maintain accurate documentation and meet performance metrics.
Work flexible hours, including evenings, to support participant schedules and programming needs.

Minimum Qualifications:

Bachelor’s degree in Social Work, Human Services, Psychology, Family Studies, or related field (or equivalent experience).
Ability to work some evening hours.
Experience supporting individuals facing barriers (fathers, young adults, justice-involved individuals).
Strong communication, relationship-building, and organizational skills.
Valid driver’s license and ability to travel within the service area.
