Full Job Description

The Program Coordinator, QIDP trains and mentors Direct Care Staff; develops implementation plans and programs for participants; facilitates contact with participants' family members; assists participants in maintaining well-managed households. Works in a team environment with other Program Coordinators and staff. Work schedule consists of some afternoon/evening hours and some weekend hours.

Principal Duties and Responsibilities

Work as a team to coordinate, support, and work with other HISway Program Coordinators or Administrative staff to ensure continuity of care for all Participants receiving services from HISway, LLC.
Being responsible for Service Plan Developement and implementation for individuals with developmental disabilities.
Knowing and enforcing IDAPA policy as well as agency policies.
Complete ISP/ Status reviews and send to the ISP team within regulated time frame.
Manage staff, participant and guardian concerns as they arise.
Work with House Supervisors/Coordinators to create staff schedule based on current ISP.
Program coordinators will have a rotating on call schedule to include overnight/weekend hours monthly.

Supervision Received:

Program Coordinators will also receive guidance from the Administrator. Supervision will be a general oversight based on compliance and an as needed basis. It is the employees’ role as Program Coordinator to ensure all duties and responsibilities are being met.

Supervision Exercised:.

Program Coordinators will ensure House Leads and Direct Care Staff are properly trained and supported in their roles. The Training Officer will assist Program Coordinator(s) in training/hiring/developing and evaluating of Direct Care Staff.

Qualifications & Skills:

REQUIRED:

Be at least 18 years of age.
Possess a Bachelor’s Degree in a Human Service field
Posess a Qualified Intellectual Disabilities Professional (QIDP) qualification
Minimum of 1-year supervised experience with population served
Ability to pass a criminal history background check
Ability to obtain and maintain current CPR/First Aid
Ability to obtain assistance with Medications for Unlicensed Personnel (UAP) card
Possess and maintain automobile insurance at all times during employment

Job Type: Full-time

Pay: $50,000.00 - $60,000.00 per year
