Full Job Description

JOB TITLE: INTAKE CASE REVIEW COORDINATOR

SUPERVISED BY: INTAKE CASE REVIEW SUPERVISOR

DEPARTMENT: INTAKE DEPARTMENT

PRIMARY RESPONSIBILITIES:

Supports the organization's mission, vision, and values by exhibiting the following behaviors: excellence and competence, collaboration, innovation, respect, commitment to our community and clients, accountability, and ownership. The Intake Case Review Coordinator is the gatekeeper for acceptance and transfer of cases from the Florida Department of Children and Families, Child Protective Investigations 24 hours a day, 7 days a week. The Intake Case Review Coordinator deals with a diverse group of important internal and external partners at all levels of the organization. Independent judgment is required to plan, prioritize, and organize workload and recommend improvements to enhance procedures.

EXAMPLE OF ACTUAL FUNCTIONS:

Demonstrate mastery of the Florida Safety Practice Model concepts, tools, and philosophy.

Apply Practice Model knowledge and use it to guide decisions and recommendations on cases and problem solving as needed when discussing/presenting cases.

Assist DCF Protective Investigators to determine services needed and locating appropriate community resources to assist families.

Collocate in DCF Protective Investigations offices in Miami Dade and/or Monroe Counties as determined by the Department needs.

Receive, review, assign, and transfer cases received from DCF/CLS/FCMA according to established protocols.

Enter and reconcile intakes received into the applicable computer database systems accurately and timely (within 6 hours unless additional time is approved by supervisor).

Verify information received in the various databases available to ensure data integrity and accurate information gathering.

Assess completion of the Family Functioning Assessment safety and risk assessment determination on every case to help ensure there is adequate information sharing and the most appropriate type of services requested based on the safety determination.

Ensure a complete and viable safety plan exists in every case presenting a danger threat.

Participates on the Decision Support Team (DST) staffing

Complete family finding efforts for every intake received

Co-Facilitate the Multi-Disciplinary Case Transfer Staffing between Protective Services and the Full Case Management Agencies

Provide follow up needs and ongoing review for timely Case Transfers/Assignment up to 30 days of intake unless additional timeframe is approved by supervisor.

Escalate case of concerns to supervisor, director, or designee

Ensure the integrity of the case assignment process by following established protocol.

Coordinate and facilitate the Case Transfer Staffing and all other staffing’s for court as per established protocol.

Participate in staffing’s as needed to include New Child, Decision Support Team, Drug Court, and Cases of Concern.

Provide daily reports on cases received to include the case information and service determination.

Assist with shift coverage (when needed) to ensure department operation maintain 24 hour a day coverage.

Develop and maintain expertise on community resources

Act as points of contacts for DCF, service providers and community partners (Law Enforcement, JAC, GAL etc.) to resolve inquiries and issues.

Answer and document incident report calls outside of business hours.

Performs other duties as necessary or assigned.

MINIMUM REQUIREMENTS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. 1. Bachelor’s degree in social work, human services, or related field.

2. At least two years of experience in dependency case management or child protective investigation services.

SPECIAL SKILLS AND OTHER REQUIREMENTS:

Knowledge of another language is helpful but not required.

Ability to receive constructive criticism and demonstrate understanding through continued growth, learning and application. • Ability to demonstrate strong interpersonal skills.

Ability to attend mandatory in-service training within expected dates as defined by the agency’s policies and procedures.

Ability to demonstrate familiarity and understanding of the Department’s Service Plan and Policies and Procedures.

Demonstrate a mastery of Florida’s Child Welfare Practice Model’s concepts, tools, and philosophy.

Adheres to all applicable Citrus FCN policies and procedures.

Ability to always collaborate and present in a professional manner.

Follows and adheres with confidentiality requirements set by federal, state, and local

Demonstrate critical thinking skills to include poise, tact, and diplomacy to resolve conflicts.

Ability to maintain good working relationships with internal and external partners.

Ability to work independently or demonstrate initiative.

Demonstrate knowledge of computer software programs that include FSFN, CJIS, LexisNexis, Stabilify, Power Point, Word, Excel, Outlook, and other computer software programs.
