Our team is dedicated to providing comprehensive hospice services tailored to meet the unique needs of each individual and their loved ones. With a focus on dignity, respect, and comfort, we strive to enhance quality of life and support families through every step of the journey.

Description:

The Chaplain provides a wide range of spiritual services to patients and families. Additionally, the Chaplain offers spiritual support to the staff and volunteers of the Company.

Job Responsibilities:

Develop spiritual care plans based on patient/family spiritual traditions and assessment results.
Provide appropriate spiritual care and initiate interventions to resolve care plan issues.
Implement spiritual care plans through personal contact or by enlisting volunteers or local clergy.
Monitor plan implementation and reassess as needed.
Maintain timely and accurate documentation of care provided.
Act as a liaison between the Company and local clergy and congregations.
Attend interdisciplinary team meetings, contribute to care plans, and provide spiritual care objectives.
Perform other duties as assigned.

Qualifications:

Bachelor’s degree in divinity or related field from an accredited seminary or institution; master’s degree preferred.
Ordination or commissioning/endorsement from a recognized ordaining body
Documentation demonstrating good standing with faith group or professional affiliation.
Completion of one unit of Clinical Pastoral Education (CPE) accredited by the Association for Clinical Pastoral Education, Inc., or in process within six (6) months of employment.
Previous experience working with an EMR/EHR system preferred.
Health care experience preferred.
Valid driver's license and automobile insurance per Company policy
Ability to travel and have reliable transportation.
Must be able to navigate various environments, including homes, hospitals, nursing homes, and assisted living facilities.
Excellent communication skills in English, bilingual in Spanish preferred.

Support Company research and educational activities.
Share expertise with co-workers, both formally and informally.
Participate in Quality Assessment Performance Improvement activities as appropriate for the position.
