What You’ll Do!

The Home-Based Therapist provides individual, family, and group therapy to children and youth (birth to age 21) who meet State criteria for Serious Emotional Disturbance (SED). Services include crisis intervention, community consultation, and education, delivered primarily in community settings such as the home or school, in accordance with Medicaid Home-Based service guidelines.

Individuals with lived experience in mental illness, intellectual/developmental disabilities, or substance use disorders are encouraged to apply.

Qualifications

Master’s degree in Social Work (MSW) with LMSW or LLMSW licensure, or a master’s degree in Counseling with LPC or LLPC licensure.
Ability to participate in the after-hours Emergency Services on-call rotation.
Ability to travel within the service area and throughout Michigan as needed.
Valid State of Michigan professional license (full license preferred; limited license accepted).

Additional Requirements

Proficiency in electronic medical record documentation.
Strong organizational and time management skills.
Valid Michigan driver’s license
Vehicle insurance meeting state requirements
Acceptable driving record
Eligibility to work in the United States
A pre-employment background check will be conducted, including criminal history (state and federal), driving record, and verification of education and employment.

Key Responsibilities

Complete Intake and Annual Assessments, Initial and Annual IPOS, Quarterly Reviews, and Progress Notes within the Electronic Health Record.
Diagnose mental health disorders using the current DSM.
Develop and maintain safety plans; ensure copies are included in the clinical record and Emergency Services materials.
Provide individual, family, and group therapy to children and families.
Create treatment plans that reflect person-centered principles and individual strengths.
Maintain documentation compliance in alignment with Wellvance policies and procedures.
