Full Job Description

Community Liaison and Client Care Coordinator

Secure Home Care LLC is seeking a highly motivated and charismatic Community Liaison and Client Care Coordinator. This is a part-time job based in Manchester, CT. The estimated hours for this position will be between 15-20 hours per week. In this pivotal role, you will be responsible for expanding awareness of our non-medical, in-home care services for seniors and driving significant referral growth.

The primary focus of this position is to cultivate strong, strategic relationships with key partners within our service areas. These partners include, but are not limited to, nursing homes, rehabilitation centers, assisted living facilities, hospitals, adult daycares, hospices, elder law attorneys, geriatric care managers, and other senior care professionals.

The ideal candidate will possess a proven track record in community outreach and excel at networking. A genuine passion for senior care is essential. We are looking for a proactive and resourceful individual who can independently identify opportunities and foster meaningful connections that directly contribute to the company’s sustained growth.

Key responsibilities:

Develop and execute marketing strategies to increase brand visibility and referrals in Manchester, CT and surrounding areas.
Build partnerships with hospitals, rehab centers, clinics, and case managers.
Conduct in-home assessments and quality assurance visits to evaluate client needs, onboard new families, and ensure the highest standard of care is maintained.
Manage and grow our online presence (social media, email newsletters, website updates).
Coordinate and optimize caregiver schedules, strategically matching personal care aides with clients to ensure reliable coverage and a "best-fit" personality match.
Promote our agency at community events, senior centers, and health fairs.
Use and enhance our existing marketing materials (flyers, brochures, etc.).
Track results and continuously improve outreach strategies.

What you need to have:

At least two years of experience in community engagement and/or business development (home care or healthcare preferred).
Strong communication and relationship-building skills.
Comfortable working independently and collaboratively.
Familiar with social media, graphic design and basic marketing tools.
Passion for helping others and making a real difference.
Reliable transportation to travel to community events, meetings, and referral sources.
