Full Job Description

Overview

Join our compassionate healthcare team as a Registered Nurse specializing in hospice care, providing vital support and comfort to patients and their families in the Salt Lake County area. This PRN (as-needed) position offers flexible scheduling for dedicated nurses eager to make a meaningful difference during critical moments. As a Registered Nurse in hospice, you will deliver expert patient care, coordinate services, and collaborate with interdisciplinary teams to ensure dignity, respect, and personalized attention for each patient. Your expertise will help ease suffering and promote quality of life in a supportive and dynamic environment.

Duties

Conduct comprehensive patient assessments to determine needs, symptoms, and care priorities.
Administer medications, manage symptom control, and provide comfort measures aligned with hospice protocols.
Collaborate closely with physicians, social workers, chaplains, and other healthcare professionals to develop and implement individualized care plans.
Educate patients and families about disease progression, medication management, and emotional support strategies.
Monitor vital signs regularly and document clinical observations accurately using Electronic Health Records (EHR) systems.
Coordinate discharge planning and facilitate seamless transitions between inpatient, outpatient, or home hospice settings.
Ensure compliance with HIPAA regulations and maintain confidentiality of patient medical records at all times.

Requirements

Valid Utah RN license with current CPR certification.
Proven experience in hospice or palliative care; critical care, ICU, emergency medicine, or hospital medicine experience highly preferred.
Strong knowledge of physiology, medical terminology, infection control policies, and health regulation standards.
Demonstrated ability to perform patient assessments, manage complex cases, and utilize EMR/EHR systems effectively.
Excellent communication skills for client interaction, team collaboration, and family education.
Ability to work independently while managing multiple patients in a fast-paced environment.
Familiarity with post-acute care practices including discharge planning and home health services is advantageous.
