Full Job Description

Job Overview
Join our dedicated team at HELP Ministries (a program of Christian Churches United) as a Case Manager for the Homeless Assistance Program (HAP), where your passion for social services and commitment to high-risk populations will make a meaningful difference. In this vital role, you will provide case management to clients eligible for HAP services, connecting them with resources and services to help them stabilize their housing, income/employment, and medical and mental well-being, along with assisting them in addressing other barriers relating to homelessness as applicable.

Reports to the Unsheltered Services Manager

QUALIFICATIONS:

Minimum of one-year experience providing individualized support in a human service setting.
Associate or bachelor’s degree in human services, social work, or related areas. Certified Recovery Specialist certification preferred or experience as a mental health support professional would be a plus.
Ability to work effectively in a team environment, as well as independently in a client field or home setting providing case management and other support, with minimal supervision.
Proficient at providing successful client support, as determined by achievement of desired outcomes, preferably for individuals or families experiencing homelessness, and including those with substance use and mental health disorders, income limitations, decreased housing choice opportunities, along with other barriers that impact successful transitions out of homelessness.
Experience working with private landlords and housing authorities a plus.
A self-starter that takes initiative in performing job responsibilities with the motivation to get things done with minimal supervision.
Ability to communicate clearly, effectively and compassionately with individuals that are sometimes under great levels of stress, along with the skills to adequately manage and deescalate highly tense situations that can sometimes occur.
Attention to detail in completing job responsibilities, especially with client case notes and other documentation, along with the ability to follow directions adequately.
Experience developing and overseeing client case management/goal achievement plans, and documenting deliverables, client encounters, session/case notes, and outcomes.
Computer data entry experience, along with basic proficiency using Microsoft Office software tools such as Word, Excel, Outlook, etc., and with the ability to learn and become proficient in new software programs and tools as applicable.
Strong affirmation of CCU’s mission, and experience and interest in working with individuals from diverse cultural, ethnic, socio-economic, and theological backgrounds.
Physical ability to carry out the above duties is essential. This includes but is not limited to: Sitting or standing for extended periods of time (2 hours or more), reaching above shoulder height, bending, stooping, and lifting up to 40 lbs., and ability to climb multiple flights of stairs.
Acceptable criminal background and child abuse history checks.
Must possess a valid driver’s license, a reliable, properly insured, and registered vehicle, with the ability to use the vehicle for occasionally reimbursable job-related travel.
Ability to work 37.5 hours/week, including occasional evening hours.

RESPONSIBILITIES:

Provide direct services to support clients experiencing homelessness. These services include but are not limited to assistance with clothing, food, hygiene products, furniture, transportation, relationship issues/family reunification, childcare subsidy assistance, daycare finding search, housing search, landlord collaboration, referral for mental health or substance use disorder services, and referrals to health and wellness resources.
Assist clients in addressing crisis situations involving social, emotional, financial, health, or other problems.
Develop goal plans to help the client reach their housing and wellness goals utilizing person-centered, strength-based methods, as well as principles of trauma informed care, motivational interviewing, and harm reduction.
Participate as an active member in the Unsheltered Services team, conducting Coordinated Entry Assessments as assigned and assisting as necessary to assure quality support for all program clients.
Work in collaboration with other team members and community providers to remove barriers to permanent housing: e.g., acquisition of identification, legal aid, benefits assistance, assistance in obtaining income (SSI/SSD), life skills acquisition
