Full Job Description

About the Opportunity

Trajectory Recruiting is partnering with a respected healthcare organization in Milwaukee County to identify a compassionate and experienced Senior Care Physician. This physician will provide comprehensive medical care to an established panel of adult and geriatric patients while collaborating with a multidisciplinary healthcare team to deliver exceptional, patient-centered care.

This is an excellent opportunity for a physician who enjoys building long-term patient relationships, managing complex chronic conditions, and making a meaningful impact in the lives of older adults.

Key Responsibilities

Provide comprehensive primary care, including preventative, acute, and chronic disease management.
Perform physical examinations, health assessments, and develop individualized treatment plans.
Diagnose and treat a wide range of medical conditions using evidence-based practices.
Order, interpret, and review laboratory tests, imaging, and other diagnostic studies.
Prescribe and manage medications while monitoring treatment effectiveness.
Coordinate specialty referrals and follow-up care as needed.
Collaborate with nurses, advanced practice providers, therapists, social workers, and other healthcare professionals to ensure coordinated patient care.
Educate patients and caregivers on disease prevention, medication management, and healthy lifestyle practices.
Maintain accurate and timely medical documentation within the electronic medical record.
Participate in interdisciplinary care planning and quality improvement initiatives.
Provide clinical oversight and supervision to advanced practice providers when appropriate.

Qualifications

Required

MD or DO from an accredited medical school.
Board Certified or Board Eligible in Internal Medicine or Family Medicine.
Active Wisconsin medical license (or ability to obtain prior to start).
Current DEA registration and National Provider Identifier (NPI).
Current CPR certification.
Minimum of one year of recent experience providing primary care to elderly, geriatric, or medically complex adult populations.
Strong clinical judgment with experience managing multiple chronic conditions common in older adults.
Excellent communication, documentation, and interpersonal skills.

Preferred

Board Certification in Geriatric Medicine.
Experience working within a multidisciplinary or integrated care environment.
