Full Job Description

Position: Hospice RN

Location: Moab, UT 84532
Shift: Days
Contract: 13 weeks with possible extension
Minimum Experience: 3 years

Job Description

We are seeking a compassionate and experienced Hospice RN to provide high-quality, patient-centered care to individuals and families during end-of-life care.

Key Responsibilities

Perform comprehensive assessments of patients’ needs and coordinate individualized care plans.
Provide pain and symptom management consistent with the hospice plan of care.
Educate and support patients and families regarding the disease process, self-care techniques, end-of-life issues, and hospice philosophy.
Collaborate with the interdisciplinary team, including physicians, social workers, chaplains, and volunteers.
Participate in the on-call rotation.
Maintain accurate and timely documentation in compliance with state, federal, and organizational policies.

Qualifications

Current Registered Nurse (RN) license in Utah or a compact state RN license.
Minimum of 3 years of nursing experience, preferably in hospice, home health, or palliative care.
Strong clinical assessment skills.
Excellent communication, interpersonal, and organizational skills.
Compassionate demeanor and ability to support patients and families during difficult times.
Valid driver’s license and reliable transportation required for home visits.
Mileage reimbursement: $0.70 per mile.
Current BLS Certification.
Able to participate in 1–3 on-call shifts per week, including approximately 3 weekends during the 13-week contract, depending on staffing levels.
Must be able to respond within 45 minutes to patient needs while on call.
Must be available to work holidays.
HospiceSoft/Careficient EMR experience preferred.

Pay: $2,300.00 - $2,430.00 per week
