We are seeking a compassionate, organized, and experienced MDS Nurse/Nurse Manager to join our leadership team. This is a full-time, benefit-eligible position ideal for a nurse who thrives in both clinical leadership and clinical data assessments.

Position Summary

The MDS Nurse/Nurse Manager plays a key leadership role in overseeing resident care, supporting nursing staff and completing the MDS process to ensure accurate assessments, care planning, regulatory compliance for maximum reimbursement.

This position combines hands-on clinical leadership with MDS responsibilities and requires strong communication, critical thinking, and organizational skills.

What You'll Do

MDS Responsibilities

Coordinate and complete MDS assessments accurately and timely
Ensure compliance with State and Federal regulations
Participate in care plan meetings and interdisciplinary collaboration
Review documentation to support reimbursement and quality measures
Monitor clinical documentation for accuracy and completeness
Assist with audits, quality improvement initiatives, and survey preparedness

Nurse Manager Responsibilities

Provide clinical leadership and oversight for residents and staff
Support, mentor, and coach CNA and Nursing staff
Monitor resident conditions and communicate changes appropriately
Assist with assessments, care planning, and interventions
Ensure high-quality resident-centered care and compliance with facility policies
Help maintain staffing coverage and assist on the floor as needed during call-ins or staffing shortages
Foster a positive, respectful, team-focused culture

Qualifications

Current RN license in the State of Wisconsin
Minimumm of 2 years MDS experience required
Long-term care or skilled nursing experience preferred
Strong assessment, documentation, and organizational skills
Ability to lead, mentor, and communicate effectively with staff and families
CPR certification or willingness to obtain within 30 days of hire
Experience with care planning, Medicare/Medicaid documentation, and regulatory compliance preferred
