Job Summary
We are seeking a dedicated and compassionate Licensed Social Worker to join our dynamic healthcare team. In this vital role, you will provide comprehensive social work services to patients across various settings, including inpatient, outpatient, hospice, and community care environments. Your expertise will help improve patient well-being by delivering personalized counseling, conducting assessments, and coordinating care plans. This position offers an exciting opportunity to make a meaningful difference in individuals’ lives while working within a collaborative and supportive team.

Duties

Conduct thorough patient assessments, including diagnostic evaluations and mental health screenings, to identify needs and develop tailored treatment plans.
Provide individual, group, and family counseling sessions utilizing evidence-based techniques such as behavioral therapy, cognitive behavioral therapy, and motivational interviewing.
Assist patients with crisis intervention and manage complex cases involving addiction counseling, PTSD care, dementia care, or developmental disabilities.
Coordinate with healthcare providers to ensure seamless case management, accurate medical documentation, and compliance with HIPAA regulations.
Facilitate intake processes for new patients and maintain detailed medical records using electronic medical record (EMR) systems.
Support patients in navigating community resources, assisted living options, or post-acute care services to promote recovery and independence.
Collaborate with interdisciplinary teams on clinical research projects related to behavioral health or mental health counseling to advance best practices.

Requirements

Valid licensure as a Licensed Clinical Social Worker (LCSW) or equivalent in the relevant jurisdiction.
Proven experience in hospital settings, hospice care, outpatient clinics, or home & community care environments.
Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and medical documentation standards.
Demonstrated expertise in child & family counseling, working with individuals with autism or disabilities, and dementia or Alzheimer's care.
Proficiency in crisis management techniques and familiarity with EMR systems for data collection and recordkeeping.
Excellent communication skills for intake interviews, psychotherapy sessions, and interdisciplinary collaboration.
Ability to handle sensitive situations with empathy while maintaining confidentiality in accordance with HIPAA regulations.
