Job Title: Behavioral Health Counselor – SA Residential and SAIOP

Job Code: HCP100/Behavioral Health Counselor 1

Shift: Full-Time (40 hours per week on average)

Job Hours: 9:00 AM to 5:30 PM or 10:00 AM to 6:30 PM - Monday - Friday with some evening, weekend and holiday hours.

Base Location: Morrow/Clayton Co. and Covington/Newton Co.

The Behavioral Health Counselor serves as a key member of the treatment team in the SA Residential Program. The counselor reports to the Program Manager and works collaboratively within a multidisciplinary team that includes clinicians and peer coaches. This position focuses on providing trauma-informed, gender-responsive care that empowers women to reclaim their lives and embark on a journey of lasting recovery.

Under direct supervision, the BH Counselor will assist with care coordination, individual counseling, orientation of new residents, group facilitation, and billing FFS individual and group sessions. Additionally, BH Clinician assists with overseeing patient flow and ensuring the center’s administrative needs are being met as needed. Assists with transitions of care in a timely manner, assists with linkage to community resources, schedules and tracks appointments with VPH providers, coordinates transportation for clients as needed.

Duties & Responsibilities:

Clinician collaborates with the client in preparing an initial individual service plan that utilizes assessment data, diagnostic formulations, identified strengths, needs, preferences and resources to develop appropriate goals and objectives.
Completes face-to-face comprehensive assessment to identify problems and service needs and develop individualized service/treatment plans.
Conduct orientation sessions with new residents – review rules and guidelines, orient to paperwork, explore linkage needs (i.e. transportation, food stamps, specialty doctor’s needs), complete documentation of session in CareLogic.
Places assessment documentation and supporting data in the individual’s medical record and establishes initial Treatment Plan.
Completes and submits initial request for treatment authorization from Medicaid, State contracts, insurers or other payers according to requirements.
The clinician provides initial program orientation, individuals’ substance use disorders. Assesses individual’s treatment needs and assists him/her to develop self-directed individual treatment goals.
Meets with individuals at established intervals to discuss progress toward goals.
Works with the treatment team to identify unmet needs, review available resources, and suggest modifications to the treatment plan.
Writes clear, concise, complete, grammatically correct, and timely progress notes that relate directly to the treatment outcomes described in the service plan.
Completes discharge summaries and other documentation and reports accurately and timely.
Assembles, analyzes, and interprets new/updated information about the client in order to reassess treatment needs.
Serve as case manager to facilitate and ensure communication, linkage, and continuity of care between clients, physicians, other organizational components, community resources and service providers.
Establishes linkages and coordination of services with community agencies.
Develops and maintains cooperative working relationships with community service providers. Informs clients and families of the availability of community advocacy and support groups.
With a focus on group dynamics and building a therapeutic milieu, counselor provides group, family, and individual counseling, psychoeducation, skills-based training, and case management to individuals and their families/significant others.
Designs counseling/education services that meet the individual needs of individuals served.
Educates families/significant others on the addiction and recovery process and facilitates family support sessions as needed.
All clinical team members rotate on weekends, and on-call schedule as needed.
Provides direct care support in the residence as needed to clients including but not limited to during intake, admissions and discharge process.
Assist in the preparation of monthly and quarterly reports on program activities, client outcomes, and operational data for internal and external stakeholders.
Contribute to the development of new initiatives and activities that support the holistic recovery of women with OUD in the WOUD program.
Performs other duties as assigned.

Minimum Qualifications:

Bachelor's degree in counseling or a related field from an accredited college or university OR Certification in Addiction Counseling in the State of Georgia (CADC or CAC) through GACA or ADACBGA or Professional Licensure in Counseling or Social Work plus education, experience, and supervision in SUD treatment.
Ability to read, write, and speak English.

Requirements/Competencies:

Must be able to lift 20 pounds.
Requires long periods of sitting/standing.
Candidates for selection must pass a criminal background check (including fingerprinting).
Must pass a pre-employment drug screen and subject to random drug screens.
Must have valid Georgia driver’s license and Motor Vehicle Record in accordance with company policy.

Note: Some positions may require a valid driver's license.
