Full Job Description

Job Summary

Join our compassionate team as an In-Home Care Provider and assistant manager with on call responsibilities, where you will deliver vital support to the team and personalized care to an individual in his own home. This role offers a rewarding opportunity to make a meaningful difference in the life of an individual with disabilities, and managing chronic conditions. Your dedication will help maintain independence, improve quality of life, and stay safe within his familiar environment. We are committed to providing comprehensive training and ongoing support to ensure your success and fulfillment in this vital role. Behavioral support experience is necessary for this position.

Responsibilities

Provide attentive patient observation to monitor health status and respond promptly to changes or emergencies and behavior crisis
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
Administer medications accurately following medical instructions and document medication records in compliance with HIPAA regulations
Support clients with specialized care needs such as ptsd and anxiety related behavior challenges
Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe home environment
Take vital signs and observe patient condition closely to report any concerns or changes to mngt
Available for on call behavior support at least 5 days a month
promote independence and well-being
Enjoy board games, card games, he is the uno king
Enjoy church and Christian worship music

Skills

Strong observation skills
Experience working in home health environments, day programs, crisis centers
Knowledge of medical terminology and procedures related to caregiving and healthcare documentation
Proficiency in documentation and daily shift notes and MAR
Ability to assist with ADLs effectively while maintaining dignity and respect for clients’ preferences
CPR certification and IHSS provider verification required for this position,
must have IHSS provider number upon interview date For background check and minimum of 7 hours a week billed through IHSS
Experience working with individuals with developmental disabilities and or PTSD and anxiety related behavior challenges experience
Competence in using mobility aids like gait belts, bar assists and walkers safely/wheel chairs effectively
Excellent communication skills for collaborating with team, client, and families and behaviorist team
Ability to handle sensitive information securely in compliance with HIPAA regulations
Flexibility for overnight employment options or scheduled shifts that meet client needs. Flexibility is required so as to fit clients needs
