Overview

SHR Talent is partnering with a healthcare client in the Fort Walton Beach–Destin, Florida area in its search for Registered Nurse Case Managers (RN Case Managers). This opportunity is ideal for experienced acute care Registered Nurses looking to transition into or further develop their careers in hospital case management. Working Monday through Friday from 8:00 AM to 4:30 PM with rotating weekends, the RN Case Manager will serve as a patient advocate by coordinating care, facilitating safe discharges, and ensuring patients receive the resources and support needed throughout their healthcare journey.

The RN Case Manager will collaborate closely with physicians, nurses, social workers, and interdisciplinary care teams to develop individualized care plans, coordinate discharge planning, navigate insurance and Medicare requirements, and promote positive patient outcomes. This role requires strong critical thinking, exceptional communication skills, and the confidence to advocate for patients while partnering with providers to ensure safe and appropriate transitions of care.

Responsibilities:

Coordinate patient care across the continuum by developing individualized care plans that support safe, efficient, and patient-centered outcomes
Perform comprehensive psychosocial assessments, discharge planning evaluations, and ongoing case management assessments throughout hospitalization
Collaborate with physicians, nursing staff, rehabilitation teams, social services, and ancillary departments to facilitate timely and appropriate patient discharges
Educate patients, families, and caregivers regarding treatment plans, rehabilitation expectations, discharge planning, insurance benefits, Medicare guidelines, and available community resources
Participate in interdisciplinary rounds, family meetings, and care conferences to ensure coordinated communication and alignment of patient goals
Advocate for patients by identifying barriers to discharge, escalating concerns when appropriate, and promoting safe transitions of care
Maintain accurate and timely documentation within the electronic medical record in accordance with organizational and regulatory standards
Collaborate with leadership and interdisciplinary teams to improve patient outcomes, reduce avoidable delays, and support quality improvement initiatives throughout the hospital
Promote compassionate, patient-centered care and serve as a trusted resource for patients and families throughout the care continuum

Qualifications:

Degree

Associate Degree in Nursing (ADN) required
Bachelor of Science in Nursing (BSN) preferred

License(s) or Certification(s)

Current Registered Nurse (RN) license required
Case Management certification (CCM, ACM, or equivalent) preferred but not required

Job Experience and/or Industry Experience

Minimum of two years of recent acute care nursing experience within the past three years required
Experience in ICU, CCU, Medical-Surgical, LTAC, or Hospital Case Management strongly preferred
Hospital-based Case Management experience preferred; however, candidates with strong acute care clinical backgrounds are encouraged to apply
Experience with discharge planning, care coordination, utilization review, or transitional care is a plus

Software Skills

Experience utilizing Electronic Medical Records (EMR/EHR), case management software, Microsoft Outlook, Word, and Excel preferred

Other Skills

Strong critical thinking, clinical judgment, and problem-solving abilities
Excellent communication and interpersonal skills with the confidence to collaborate effectively with physicians and interdisciplinary teams
Highly organized with the ability to manage multiple patient cases simultaneously
Strong patient advocacy skills and commitment to safe discharge planning and continuity of care
Ability to explain complex medical, insurance, and discharge planning information clearly to patients and families
