Square Medical Group’s (“Square” or “SMG”) mission is to deliver enduring, material improvements in both clinical and wellness outcomes to patients addressing co-occurring challenges with substance use disorder (SUD) and behavioral health (BH). We do this through delivering a specialized continuum of outpatient care of 1,000+ units/week to a population of ~5,000 Members across Massachusetts: medication management, in-person and virtual therapy for individuals, couples, and families, in-person and virtual intensive outpatient and partial hospitalization programs (IOP/PHP), transcranial magnetic stimulation (TMS), in-house laboratory services, as well as, residential services.

Program Overview
SMG operates a fully integrated continuum of behavioral health and substance use disorder services designed to treat co-occurring conditions through coordinated, multidisciplinary care. The Director of Clinical Services provides clinical oversight across this continuum, with particular emphasis on three core delivery environments: (i) Co-Occurring Enhanced Residential Rehabilitation Services (COERRS) residential programming (level ASAM 3.1), which serves as a structured, 24/7 therapeutic setting focused on stabilization, life skills development, and community reintegration; (ii) Intensive Outpatient and Partial Hospitalization Programs (IOP/PHP), which deliver structured, group-based and individualized treatment at higher levels of outpatient acuity; and (iii) individual therapy services integrated within medication management as well as a standalone service line, ensuring that psychiatric care and psychotherapy are delivered in a coordinated, evidence-based manner consistent with best practices for co-occurring disorders. These programs collectively serve a high-acuity population with predominantly complex, co-occurring needs, requiring tight coordination across clinical disciplines and levels of care.
Role Summary
Reporting directly to the Chief Strategy Officer for all programmatic and data workstreams and to the Chief Medical Officer (CMO) for clinical issues, the Director of Clinical Services is the clinical leader responsible for supporting the maintenance and implementation of regulatory and organizational clinical policies and procedures across programming. The Director of Clinical Services does not independently own compliance or site-level implementation; rather, the role partners closely with programmatic leaders, compliance department, and executive leadership to ensure clinical services, workflows, documentation practices, and supervision structures are aligned with SMG’s clinical philosophy, evidence-based practices, and applicable BSAS, DPH, state, federal, payor, and organizational requirements. Where policies and procedures are not documented or need updating, the Director of Clinical Services collaborates with programmatic leaders and Compliance to develop, distribute, and support training on standardized solutions across the organization.
While the Director of Clinical Services maintains responsibility for clinical quality, consistency, and adherence to SMG’s evidence-based care model across all programs, the role does not replace programmatic or administrative leadership at the site level. Instead, it establishes the clinical framework, standards of care, and supervision necessary to ensure that each program, across residential and outpatient settings, operates cohesively within a unified clinical philosophy. This includes defining clinical protocols, overseeing supervision structures, and ensuring integration between service lines, while partnering with Program Directors and executive leadership to operationalize delivery within each care setting. Ultimately, the Director of Clinical Services is responsible for leading Square Medical Group’s data program to track, measure, and improve patient outcomes across the continuum of care.
This role also ensures that recruiting, onboarding, and clinical data systems are accurate, consistent, and scalable. The Director of Clinical Services will also directly supervise (or indirectly through other supervisors) all clinicians, ensuring accountability workflows and service levels in coordination with program directors. In addition, this role will help streamline and align all clinical technology platforms to ensure data integrity, documentation consistency, and proper system utilization.
Key Responsibilities
1. Clinical Quality & Standards of Care
a. Define, implement, and continuously refine SMG’s clinical model across COERRS, IOP/PHP, and outpatient therapy
b. Ensure all services align with evidence-based practices for co-occurring disorders (e.g., CBT, DBT, MI, trauma-informed care)
c. Maintain and strengthen existing clinical documentation requirements across programs, ensuring required documentation practices are preserved, standardized where appropriate, and consistently aligned with BSAS, DPH, state, federal, payer, and organizational requirements; establish related clinical protocols and case management standards and drive continuous quality improvement initiatives.
d. Maintain availability for urgent clinical matters, including crisis situations and emergencies, and provide timely guidance or intervention as needed to ensure patient safety and continuity of care
e. Own clinical outcomes performance, including retention, engagement, and program completion rates, and drive improvement initiatives based on data
2. Clinical Process Design, Documentation & Compliance
a. Partner with Program Directors and clinical leaders to document and standardize all core clinical workflows (e.g., intake, assessment, treatment planning, service delivery, discharge) across COERRS, IOP/PHP, and outpatient services
b. Build scalable SOPs, clinical playbooks, and role clarity that enable consistent execution and replication across locations
c. Partner with Compliance and programmatic leaders to ensure clinical staff meet applicable BSAS, DPH, state, federal, payer, and organizational requirements, with compliance checkpoints embedded directly into workflows and documentation standards.
d. Translate workflows into EMR systems (e.g., InSync) and drive training, adoption, and continuous improvement through audits, KPIs, and supervision structures to enable consistent replication across sites.
3. Patient Outcomes Data Program
a. Serve as the clinical leader for the implementation and ongoing management of the PROEM outcomes measurement platform across all ASAM 3.1 residential programs and IOP/PHP programs
b. Oversee the administration of PROEM assessments, including measures of anxiety, depression, substance use urges, alcohol use disorder, social determinants of health, daily functioning, and therapeutic alliance, in collaboration with Program Directors, Clinical Supervisors, and Case Managers
c. Ensure consistent and complete data collection at admission, every 30 days, and at discharge across all applicable program locations
d. Monitor accumulating PROEM results and generate performance reports, including charts and trend analyses, once sufficient sample sizes are reached
e. Facilitate a recurring Users Group with Case Managers and Clinical Supervisors to address implementation issues, provide feedback, and drive continuous improvement of the outcomes program
f. Integrate PROEM data with clinical technology platforms (e.g., InSync, PowerBI) to inform clinical decision-making, performance management, and program quality improvement
g. Leverage patient outcomes data to substantiate program performance for Value-Based Care contracting and risk-based payer negotiations
4. Clinical Oversight Across Continuum of Care
a. Provide direct clinical oversight of COERRS programming, including group curriculum, treatment planning, and resident care
b. Ensure consistency and integration of care across IOP/PHP and outpatient therapy services
c. Oversee clinical decision-making frameworks to ensure appropriate level-of-care placement and transitions
d. Maintain alignment between medication management and therapy services to support integrated treatment delivery
5. Clinical Supervision & Workforce Development
a. Provide (or oversee) clinical supervision for clinicians, case managers, and interns
b. Establish supervision standards, cadence, and documentation requirements across programs, consistent with BSAS and DPH regulations
c. Support recruitment, interviewing, onboarding, and retention of high-quality clinical staff
d. Develop training programs to ensure competency in SMG’s clinical model and evidence-based practices
6. Regulatory Compliance & Risk Management
a. Ensure all clinical services meet state, federal, and accreditation requirements (e.g., licensing, HIPAA)
b. Oversee clinical documentation quality, including audits of assessments, treatment plans, and discharge planning
c. Develop and maintain clinical policies and procedures in coordination with the Chief Medical Officer
d. Escalate and manage clinical risk issues, including crisis intervention and incident review
7. Clinical Operations & Cross-Functional Alignment
a. Partner with Program Directors to align clinical expectations with day-to-day program operations
b. Collaborate with medical leadership to ensure seamless integration of psychiatric and therapeutic services
c. Support coordination with billing, intake, and administrative teams to ensure clinical activities translate to authorized and reimbursable services
d. Drive clarity in roles, responsibilities, and workflows between clinical and non-clinical teams
8. Data, Documentation & Clinical Technology
a. Ensure accuracy, consistency, and completeness of clinical documentation within EMR system (e.g., InSync)
b. Partner with leadership to define and track key clinical KPIs (e.g., engagement, retention, outcomes)
c. Support optimization and standardization of clinical workflows within technology platforms
d. Utilize data and reporting (e.g., PowerBI) to inform decision-making and performance management
9. Program Development & Strategic Initiatives
a. Support expansion and scaling of SMG programs, particularly COERRS residences
b. Identify and implement new evidence-based practices to enhance the clinical “brand”
c. Contribute to strategic planning and corresponding marketing initiatives, including new service lines, partnerships, and program enhancements
d. Assist in developing family programming and other wraparound services across the continuum
10. External Relations & Clinical Representation
a. Represent SMG in interactions with referral sources, regulators, and community partners
b. Build relationships with academic institutions to support internship and training pipelines
c. Maintain current knowledge of emerging research and best practices in Co-Occurring Treatment for Adults and translating findings into programmatic practices
d. Support marketing and business development efforts through clinical credibility and thought leadership

Qualifications
- Doctoral degree (PhD, EdD or PsyD) in psychology, Master’s degree in social work, or Master’s degree in mental health counseling.
- Current Massachusetts licensure as a Clinical Psychologist or Licensed Independent Clinical Social Worker in good standing.
- Experience with BSAS/DPH-regulated SUD programming preferred
- Experience with co-occurring disorders and residential levels of care
- Experience with EMR documentation and clinical audit processes
- 10 years minimum experience in provision of services to adults with co-occurring disorders, across a continuum of services
- At least 5 years of supervisory and leadership experience.
