Job Summary:
We are seeking a highly skilled and dedicated Pharmacist to join our team. As a Pharmacist, you will be responsible for providing exceptional patient care by ensuring the safe and effective use of medications. Your expertise and knowledge in pharmacology will play a vital role in improving patient outcomes and promoting overall wellness.
Responsibilities:
- Conduct patient assessments to determine appropriate medication therapy
- Collaborate with healthcare professionals to develop comprehensive treatment plans
- Provide medication counseling to patients regarding proper usage, potential side effects, and drug interactions
- Monitor patients' response to medication therapy and make necessary adjustments
- Ensure accurate and timely dispensing of medications
- Perform medication reconciliation to prevent adverse drug events
- Conduct utilization reviews to optimize medication therapy
- Administer vaccines and review patient's immunization history
- Collaborate with pharmacy technicians to ensure accurate medication preparation and dispensing
- Maintain accurate patient records and documentation
Qualifications:
- Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy
- Valid pharmacist license in the state of MA
- Strong knowledge of pharmacology, anatomy, and physiology
- Experience in patient assessment and medication management
- Proficiency in utilizing pharmacy software systems for prescription processing and inventory management
- Excellent communication and interpersonal skills for effective patient counseling and collaboration with healthcare professionals
