Full Job Description

We have a current opening for a Youth Mentor/Case Manager position within our Adolescent Diversion Program (ADP). ADP is operated in conjunction with Wood County Human Services Department, located in Wisconsin Rapids, WI.

The ADP Youth Case Manager is to provide a system of care that is youth-centered with the needs of the youth and family dictating the types of services provided, to assist in keeping the family together and preventing youth justice involvement or adjudication.

To be effective, the case manager will work with youth in their homes and communities, incorporating the entire family to provide wrap around services.

This position performs ongoing, supportive case management services to youth. The case manager is responsible for mentorship, technique-building and family systems case management for program participants, working collaboratively to develop a strengths-based prevention plan that assists youth in increasing resiliency, positive emotional vulnerability and community building.

This position is youth-driven and focuses on developing youth strengths, promotes positive relationships and connections for youth participants, and provides the support and follow-up services needed to address the range of challenges participants confront.

Minimum Training and Experience

A Bachelor’s Degree in Human Services, Social Work, Sociology, Health and Wellness or related field that provides the necessary skills to meet the expectations of the position.
Knowledge of human behavior, motivation, crisis intervention, family and group dynamics, money management, and organizational skills are necessary.
Interviewing and assessment skills are required.
Participation in State or Agency training is required.
Basic everyday living skills are needed, as is the ability to understand and follow oral and written directions.
Reading, writing, adding, and subtracting skills are needed.
Must have a valid Wisconsin Driver's License and access to transportation to travel to various work sites.
Basic office equipment used: personal computer, projector, printer/copier and telephone.

Ability to work positively when children express reactive behaviors and actions
Proven background experience in working with children who may express heightened trauma response
Ability to navigate between family experiences and child behavior with strength-based approach
Strong vision of hope and positive expectation and outlook when engaging all parties involved
Schedule is non-traditional, but flexible and based around clients’ availability. Since case managers are heavily involved with each family, long hours may be required.

Work Hours: 40 hours per week. This position requires a flexible schedule to meet the needs of the youth and family. Currently most ADP mentors/case managers start work in the late morning and work with youth until early evening, as late afternoon and early evening hours generally work best with families. Occasional Saturdays may be worked, if needed for a family but the position generally works Monday - Friday. The case manager must have a driver's license and a personal vehicle. All business travel/mileage is reimbursed at the current IRS rate.

Employment Status: Permanent, full-time (40 hours per week) Must be flexible to meet family needs with adjusted schedule that may include some evenings.

Starting Wage: $27.05
