JOBDESCRIPTION

Job Title: Case Manager Reports to: Administrator/DPS

Job Objectives: To assist the Administrator in the daily operations of the

Home following the Department of Health Regulations and the Policies and Procedures of the Home. To encourage participation in activities and daily activities of daily living, provided to enhance the residents quality of life. Ensure a friendly, safe and secure environment. Be a vital member of the facilities administrative team

Responsibilities and Duties:

1. Maintain strict adherence to the Department of Health Regulations. Will ask for assistance or clarification when in need of help from the Administrator/DPS.

2. Communicate respectively and effectively with residents and appropriate family members regarding any information as it pertains to each individual resident.

3. Supervise, monitor and assist in training to all Personal Care Assistants.

4. Monitor and manage the Daily Communication Log, ensuring all Personal Care Assistants are documenting daily, on each shift, in the task sheets.

5. Assist in assessments, interviews, and evaluates prospective residents to determine appropriateness for admission.

6. Orients new residents to the daily routine.

7. Identifies adjustment problems and assists the resident to resolve such problems.

8. Assists the resident to maintain family and community ties and develop new ones, develops new interests and forms solid support systems.

9. Contacting the residents primary care physician or ancillary physician for any needs specific to the residents, documenting as necessary.

10. Coordination and performing services as necessary to provide residents’ with an optimal living situation taking care to enhance their autonomy and socialization skills.

11. Provides one on one support for residents related to emotional, behavioral, and social concerns.

12. Coordinates medical appointments and other ancillary services, ensuring attendance to appointments and make changes as needed.

13. Maintains accurate and timely records of case management interventions, appointments, recommendations and follow up in each resident’s chart.

14. Maintains case management documentation accurately and in a timely manner annually and as needed related to changes in a residents condition or status.

15. Documents in charts any medical related problems or needs.

16. Maintains resident’s records and confidentiality while carrying out guidelines of case management.

17. Conduct, initial and annual Mental Health Assessments

18. Conducts initial and annual case management evaluation

19. Incident Reporting as per DOH regulations

20. Assists residents and families in need of alternate living arrangements to make and execute safe transfers and/or discharge plans.

21. Responsible for biannual departmental Quality assurance audits are completed and reported to Program Director.

22. Updates the Program Director on routine basis regarding residents needs/changes.

23. Actively advocates on the residents behalf, at all times.

24. Provides respect and courtesy, dignity, and confidentiality in a professional manner at all times with guests, residents, and staff.

25. Represents the facility in a professional, courteous, and respectful manner when working with the public, potential move-ins, staff, etc

26. Additional duties as many be assigned by Administrator

Job requirements and Work conditions:

1. Clean well-lit, well-ventilated working environment in controlled temperature in common areas.

2. In and out of apartments that each resident individually controls their own heat and A/C temperature.

3. May be exposed to body fluids which could contain disease such as HIV, Hepatitis B, etc. (Is not required or permitted to do injections of any kind)

4. Required to read, interpret and transcribe basic medical orders accurately.

5. Ability to multi-task assignments in a fast paced environment.

Qualifications/Experience:

· Bachelor’s degree from an accredited college or university with major work in human services or service delivery and one year of full-time experience in the provision of services to a frail adult population

· or an associate degree from an accredited college or university with major work in a human services or service delivery and three years of full-time experience in the provision of services to a frail adult population

· One year of successful experience working in a care management capacity in a health care setting with DOH approval.

· Able to speak, understand, write and read English

· Emotionally, physically and mentally able to provide assistance to the population served by the HOME.

· Successful experience communicating and working with older adults, the general public and with other team members.

This is a full time in person case manager position for Wavecrest Assisted Living

Pay: $60,000.00 - $65,000.00 per year
