Full Job Description

Interborough Developmental and Consultation Center is now hiring Case Manager/ Care Coordinators.

Health Home Case Management Program which offers care coordination and resources for adults with two or more chronic conditions throughout New York City.

The goal of the Health Home program is to make sure its members get the care and services needed. Under the general supervision of the Program Director/designee, functions as part of an interdisciplinary team assisting assigned individuals in developing and maintaining viable living, working and social situations in the community by obtaining needed medical, social, psycho-social, educational, financial, vocational and other services.

QUALIFICATIONS:

BA/BS Degree Required (MSW Preferred)
1-2 years of experience in healthcare, social work, case management, discharge planning or third party payer experience preferred.
Strong written and verbal skills
Excellent computer skills are necessary, demonstrated ability to use word-processing, or data base programs.
Knowledge of community resources, and entitlements/benefits.
Good organizational and interpersonal skills.
Bilingual Spanish/ Russian encourage to apply

Hourly Rate: $26.00-$28.00

This position requires the candidate to be in the field about 75% of the time.

RESPONSIBILITIES

● Conducts initial and ongoing assessments of assigned clients to document strengths, needs, goals, and resources.

● Maintains effective communications with clients, primary care physicians, substance abuse, and mental healthcare providers, family, collateral resources and other Agency staff on behalf of clients.

● Maintains documents, records, statistics, and other related reports in an organized, timely, and accurate manner as per policy and procedure.

● Coordinates care planning with other providers of services/ resources to ensure goal directed, collaborative care, including care transitions.

● Works as part of a Care Coordination team; attends and participates in team meetings to provide input/feedback around psychosocial and medical conditions/comorbidities to review client status, update plans and goals, review outcomes to further program goals.

● Provides face-to-face engagement and service planning in the field.

● Acts as a linkage to community services including medical, behavioral, residential, entitlement and any other needed services per interdisciplinary care plan.

● Monitors overall service delivery to clients to ensure coordination and continuity.

● Provides crisis intervention and follow-up.

● May be assigned other tasks and duties reasonably related to the job responsibilities.

● Complete all notes, BSQ’s and other essential documentation on a daily basis.

● Maintaining the greatest degree of independent living. The majority of duties are often performed independently under general supervision.

● Works closely with the interdisciplinary care team including PCP, psychiatrist, therapist, residential services, substance abuse treatment program, etc.

● Provides input to providers/client/family for written individualized care plans.

● Attend care coordination meetings, collaborate with out-reach staff and external care coordinators, assess consumer needs and discuss/advocate for needed services.
