Job Title: Care Manager (Guardianship)

Reports To: Co-Directors of Guardianship

Employment Type: Full-Time

About Family Service Society of Yonkers (FSSY):Family Service Society of Yonkers is a community-based nonprofit organization that has served residents of Yonkers, Westchester, and the Bronx for over 140 years. We provide comprehensive services to individuals and families of all backgrounds and ages, including the largest and most highly regarded Guardianship program in the region.

We are seeking a dedicated Case Manager to join our Guardianship Program. This is an excellent opportunity to make a meaningful impact in the lives of elderly clients while working with a collaborative and mission-driven team.

Position Overview:

Under the supervision of a team of social work professionals, the Case Manager will coordinate services and provide support for elderly clients, including those with dementia or Alzheimer’s disease. This position involves working closely with families, the court system, healthcare providers, and community agencies. Approximately 50% of this role involves field work, including home and facility visits to clients.

Key Responsibilities:

· Coordinate services for elderly clients in the Guardianship Program.

· Collaborate with family members, courts, hospitals, social services agencies, nursing homes, and other elder care facilities.

· Conduct regular field visits to clients to assess needs and ensure appropriate care.

· Develop, implement, and monitor individualized care plans.

· Maintain accurate documentation and records in compliance with program requirements.

· Advocate for clients to ensure access to services and resources.

· Participate in team meetings, trainings, and case reviews as required.

Qualifications:

· Bachelor’s degree in Social Work, Social Services, Human Services, Counseling, or a related discipline

· New graduates are encouraged to apply

· Bilingual (English/Spanish) preferred

· Proficiency with Microsoft Office Suite and basic computer applications

· Valid driver’s license and access to a personal vehicle for client visits
