Full Job Description

Job Summary
Join a top-tier healthcare organization offering a fully remote RN Care Manager role after initial training in Lower Manhattan. This position comes with a competitive salary, comprehensive benefits, and the opportunity to make a meaningful impact by coordinating care for members across diverse healthcare settings.

Responsibilities:

Conduct telephonic and occasional in-home assessments of members’ medical, psychosocial, physical, and spiritual needs.
Develop, implement, and monitor Person-Centered Service Plans (PCSP) using UAS-NY data.
Respond to changes in member condition or sentinel events by updating care plans and communicating with the interdisciplinary team.
Collaborate with PCPs, specialists, member representatives, and internal care teams to ensure continuity of care.
Educate members on disease processes, preventive health, and quality-of-life strategies.
Participate in interdisciplinary care team meetings and present member cases.
Utilize benefit structures and community resources to meet member needs.
Ensure compliance with CMS and DOH guidelines.

Qualifications:

Graduate of an accredited nursing program; BSN preferred.
Current RN license in New York State.
CCM certification preferred.
3–5 years of nursing experience in long-term care, managed care, home care, geriatrics, or hospice.
Prior experience in interdisciplinary service delivery environments.
Working knowledge of Medicare and Medicaid regulations.
Strong communication, organizational, and care coordination skills.
